Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Case Management- Incident inspection at the above facility on 05/08/24 at 01:05 p.m. LPA met with licensee, Sandra Thamas who guided LPA on a tour of the facility. Licensee’s assistant Marbelis Tomasen Melendez was also present during this inspection. Census was taken.
The purpose of this inspection is to follow up on an unusual incident report that was reported to the Department on 05/06/24. On 05/03/24, child 1 (C1), was getting ready to walk down the stairs to go play at the back yard when C1 rolled 3 steps down. Per licensee, she was standing at the top/1st step of the stairs and C1 on the second step waiting for the other older children to gather so everyone can proceed to the backyard. According to the licensee, C1 didn't wait and proceeded to walk down the stairs. Licensee observed C1 rolled 3 steps down; however, licensee managed to grab C1 preventing C1 from rolling further down the stairs. It is unknown if C1 missed a step or tripped with their own feet. Licensee assessed C1 and did not observe any bruising, bumps, scratches, or any visible injuries. Licensee notified parents of C1 and according to the licensee, C1 was not taken to the doctor. C1 returned to the facility on 05/06/24. C1 has been observed active with no complains of any pain. Per licensee, older children who can walk and are well balanced must wait for licensee or assistant, grab the rail, and walk down the stairs. Licensee also stated that younger children are not allowed to walk down the stairs; however, they are carried by licensee or assistant. Per licensee, younger children are waiting inside the play pen. Licensee also stated that 1 adult must remain with the children who are waiting at the bottom of the stairs while the other adult is bringing the younger children down. LPA observed licensee has (2) well mounted child proof gates at the top of the stairs preventing access to the second level of the home. LPA tested the steel rail on the stairs and was deemed sturdy. LPA walked on the stairs and observed them to be well maintained, free of debris, in good condition. LPA did not observe anything that could be a potential trip hazard. LPA took pictures of the stairs and rail.
