On 08/17/2023, at 8:38AM, Licensing Program Analyst (LPA) Thelma Razo conducted a Case Management - Other visit to follow up on information received in the Regional Office. A COVID 19 risk assessment was conducted prior to entering the facility. LPA met with Assistant Director Cindy Stansbury during the visit. Also present in the facility were 7 infants and 5 staff. Facility was within ratio & capacity.

A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the facility representative, Cindy Stansbury.
