On January 3, 2025 at 12:00 pm, Licensing Program Analyst (LPA) Priscilla Ochoa conducted an unannounced annual inspection. LPA was greeted by one guard at the back entrance of the facility, who then contacted the Director Lauren Friedman who guided LPA on a tour of the facility. This is an Infant program that operates Monday through Friday from 7:30 am to 6:00 pm. Per director there are 17 infants enrolled in the program and LPA observed 13 infants in care.

This facility is located on the premises of Westside Jewish Community Center and Preschool #191800750. LPA observed the infant program to be physically separate, both indoors and outdoors from the Westside Jewish Community Center and Preschool program.

At 12:15 pm, director guided LPA on a tour of facility. This Infant program consist of 1 classroom which was inspected. LPA observed 13 infants and 7 staff members. There are age-appropriate toys and supplies for children in the classroom and in good repair. Cribs and napping equipment were observed. LPA observed portable cribs that meet Consumer Product Safety Commission safety standards. Mattresses have an appropriate size mattress with fitted sheets. Cribs were observed to be free of loose items. Cots were available for infants who are able to climb out of a crib. Bedding was inspected for good condition, appropriate storage, and cleanliness. Sleeping area is physically separated and cribs do not limit staff’s ability to watch infants at all times. Per director, bedding is changed daily and sent home every Friday to be washed. LPA observed the diaper changing table to be at least one inch thick, with raised sides of at least 3-inches, and within arm’s reach of the sink. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All floors are clean and safe. Sufficient individual storage space for children was observed and each child has a cubby.

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. The classroom was inspected to ensure that the floors have a surface that is safe and clean. All materials and surfaces accessible to children are toxic free.

All food preparation areas and food storage areas are kept clean and are free of litter, rubbish, and rodents and/or any other vermin. This facility provides all meals for infants in care. LPA observed the fridge had bottles and food that were labeled and stored individually for each infant in care. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects and rodents.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children.

There is drinking water accessible to the children in care via personal water bottles and water dispensers through out the facility.

First Aid supplies are available and complete. At this time, the Director's Office is used as an isolation area. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted

12:35 pm. the outdoor playground was inspected. Equipment in the play yard is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. LPA observed playhouses, small climbing equipment, and play mats. There is adequate shade in the play yard via snail shades.

Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision.

LPA observed a hardwired smoke detector. LPA also observed a functioning carbon monoxide detector during the inspection. LPA observed a fully charged fire extinguisher that was serviced on 06/04/2024. Emergency disaster drill was last conducted on Sep. 24, 2024.

The facility uses Playground application to sign in and out infants. Parents scan a barcode that directs them to sign in/out their child. Teachers also have a paper copy of the sign in/out sheets in the classroom. LPA reviewed Sign in and out sheets to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Child records were deemed not complete. 4 out 4 infants did not have LIC995 in their file for LPA to review.

Staff records were reviewed to ensure Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Staff records were deemed not complete. 2 out of 3 staff did not have LIC 503 available for LPA to review. Please see attached LIC 809-D.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.

This facility does provide Incidental Medical Services (IMS). LPA observed medication storage, equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP

LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observations and record review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted, and a copy of the report was provided to Director, Lauren Friedman.
