25 | PRELICENSING INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an announced pre-licensing inspection to the above facility on 12/27/2022. LPA arrived at the facility at 11:45 AM and met with Ira Ohanjanyan, Applicant who guided analyst on a tour of the facility. During this inspection Lilit Ginosyan, applicant's daughter in-law was also present in the home. Pre-licensing Entrance Checklist (LIC9280) was provided to applicant, Ira Ohanjanyan. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per applicant operation hours will be Monday to Friday, 7:30 a.m. to 10:00 p.m.. Applicant states she will care for children 0-12 years old.
Applicant previously was licensed at 732 Portola Avenue, Glendale, CA 91206 with facility number 198019750. Fire clearance for Large Family Child Care Home was granted on 12/14/22.
Applicant did not submit Property Owner/Landlord Consent (LIC 9149) form.
All areas identified on the facility sketch were inspected. This is a one story home located on the first level. The home consists of 3 bedrooms, 2 restrooms, living room, dining room, kitchen, laundry room, basement, front yard, patio (deck) in the backyard, backyard (fenced), and detached garage. There is a fireplace in the living room which has been blocked off to prevent access to children. Per applicant, parents will enter the home through the main entrance which leads to the living room.
Areas that are accessible to children are as follows: Bathroom in the hallway, living room, dining room, one bedroom adjacent to the living room, patio (deck) and backyard (fenced).
Areas off limits based on facility sketch submitted to children and parents include: Two bedrooms, one bathroom in the master bedroom, kitchen, laundry room, basement, front yard, and detached garage in the backyard.**Rooms that are off-limits need to be made inaccessible during operating hours**
The applicant does understand that licensing staff may have access to off-limit areas during inspection visit if necessary.
