25 | PRELICENSING INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an announced pre-licensing follow up inspection to the above facility on 12/28/2022 to inspect the basement. LPA arrived at the facility at 1:20 PM and met with Ira Ohanjanyan, Applicant who guided analyst on a tour of the facility. During this inspection Lilit Ginosyan, applicant's daughter in-law was also present in the home.
At 1:35 p.m. Michele Lynch, Single Family Homes Agent and Supervisor from Trumark Real Estate management arrived to the facility who opened the basement door. LPA observed there are three stairs and a ladder to go to the basement. LPA did not go in the basement as far as there was not secured stairs. Lilit Ginosyan, applicant's daughter in-law took pictures from basement and LPA did not observe any hazards in the basement.
Per applicant, basement belongs to the landlord and will be kept locked all the times. Per applicant, she does not have access to the basement.
A large family child care license will be granted upon receipt of proof of corrections for the above. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license.
The applicant’s signature on this report acknowledges that they have signed the Application for a Family Child Care Home License (LIC 279) under the penalty of perjury that the statements on the application and any attachments are correct.
