25 | On 4/30/2026, at 1PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Annual/Random inspection to the above facility. Upon arrival, LPA met with Staff members / Shoghakat Tadevosyan, Narek Sedrakyan and Veronika Boshyan, who assisted with the visit. LPA disclosed the purpose of the visit. Entrance Checklist – Family Child Care Homes was provided. Narek Sedrakyan guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. The Licensee / Kima Sedrakyan arrived at the facility sometime after and assisted with the visit. There were six (6) day care children present during today’s inspection. Licensee states that there are currently 11 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Friday, from 6AM - 10PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 3/13/2026.
During today's visit, LPA conducted a walk through of the physical plant only. This is a single story home which consists of a living room, kitchen, 1 bedroom, 1 bathroom, back yard, side yard, front yard and a laundry room (located in the back yard). Areas that are accessible to children are as follows: Living room (which is used as the main day care room), bathroom and side yard. The side yard has grass and concrete with age appropriate toys. Per Licensee, off-limits areas are as follows: Bedroom, kitchen, front yard, back yard and laundry room. The off-limits rooms are made inaccessible by child safety gates and child proof door knobs/locks.
All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. There is a working telephone maintained in the home. LPA observed age appropriate toys, free of loose and sharp parts. LPA observed electrical cover outlets all through the home. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. The Licensee states that there are no poisons in the
home. The Licensee does understand that poison must be locked with a key or combination lock.Pg. 1 of 2 |
