32 | Based on the incident reports and staff members, extend of injuries scrapped knee and elbow. Per staff , incidents occurred when children run , drive bike or car and forget about lump in that area . During the children file review, LPA observed multiple incident (ouch) reports for different children . LPA verified with staff , who created the ouch report the area of the incidents . LPA contacted parent regarding incident reported by the complainant . Per parent , C3 and C4 had a recent incident outside in outdoor play area . Director stated ,ouch reports completed by the staff members who observed the incidents . Per Director , often incidents in outdoor are due to the sand on the pavement which causes children slip and fall .
Based on LPA`s observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division & Chapter one ), are being cited on the attached LIC 9099D.
Exit interview conducted with Facility Representative ,Emily Laguna . The Notice of site visit , report and Appeal Rights were explained and provided to Facility Representative on 11/21/24 date.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.
