Licensing Program Analyst (LPA), Anomeh Eivazian, conducted an unannounced case- management inspection to the above facility on 10/01/24 at 10:00 AM. LPA met with Silvana Zadoorian, licensee who guided LPA on a tour of the facility. During this inspection 11 children were present in the home playing in the living room, three were infants. Also, Ofik Ellyassi, licensee’s assistant was present in the home.

Based on licensee's disclosures on 10/01/24, Elena Zakaryan and Miro Aboolian live in the detached guests home in the backyard. Elena Zakaryan has criminal record clearance on Licensing Information System (LIS). Miro Aboolian did not have criminal record clearance on Licensing information System (LIS). Per licensee, Miro Aboolian, licensee's son lives at this address since December 2023.



The following are being cited in accordance to Title 22 of the California Code of Regulations. Please refer to 809D for cited deficiencies.

LPA Anomeh Eivazian informed licensee, Silvana Zadoorian that this report dated 10/01/24 documents 1 Type A citation. Type A citation which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Eivazian informed the licensee to provide a copy of this licensing report dated 10/01/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.


The notice of site visit was posted where the parent/guardian of children enter and exit the facility. This notice shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty.

Per licensee, her son Miro Aboolian will complete livescan today and a copy of livescan form will be submitted to LPA Eivazian via email by 10/02/24.
$500 Civil Penalty was accessed during this inspection.

During this inspection licensee disclosed that her son, Miro Aboolian lives at this address in the detached guests house since December 2024 and did not have criminal record clearance on file. This poses an immediate health, safety and personal right risk to the children care.
