13 | On February 27, 2026, Licensing Program Analyst (LPA) Monica Ruiz conducted an unannounced complaint investigation to deliver findings and met with Licensee Nilvia Nunez, to whom the reason for the visit was explained. There were 5 children present during today’s visit.
During the course of the investigation, LPA Ruiz interviewed the Licensee, staff, children, parents, neighbors, reviewed facility records, including but not limited to the children’s roster, parent and employee handbooks, staff and children’s files, photos and obtained other pertinent documents.
LPA Ruiz did not interview the RP because complaint was filed online, anonymously. RP left no contact information and wishes to remain anonymous.
According to the Reporting Party (RP), Licensee did not ensure that adequate supervision was provided to children in care. In the complaint email, RP indicates that from 7am - 4Pm there is loud crying, loud screaming, and Licensee leaves home to park cars and leaves children unattended.
