On 2/27/2024 at 10:00 am, Licensing Program Analysts (LPAs), Crystal Green and Shushanik Safaryan conducted an announced Pre-Licensing inspection for a new infant license. Upon arrival, LPAs met with Applicant Representative, Judy Cashell. The applicant, Pacific Clinics, has requested to be licensed for a new infant program with a capacity of 8 ages 0 to 18 months along with a toddler option component capacity of 19 ages 18 to 36 months. Hours of operation will be Monday to Friday from 7:30am to 5:30pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representative stated that they will keep it in a locked medicine box located in the Director Office. A fully equipped first aid kit is also kept in each classroom near the exists. There is an operational carbon monoxide detector on site located throughout the facility. All required licensing documents were observed posted on the wall in each classroom. Children will be signed in and out at the entrance of each classroom.

LPAs continued to tour the facility and measured all indoor and outdoor activity space. This Infant program with toddler option component will operate out of 3 classrooms; Classroom 1, 2, and 3. Classroom 1 will be designated for Infants ages 0- 18 months. Classroom 2 and 3 will be designated for toddlers ages 18- 36 months. Total indoor activity space for Infants measured 845.44 square feet which is sufficient to accommodate the requested capacity of 8 infants. Total indoor activity space for Toddlers measured 628.87 square feet which can only accommodate a capacity of 17 toddlers.

LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including small tables, chairs, cubbies, 2 cribs, napping cots for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. Per Applicant Representative, Judy Cashell, the infant program will only enroll a maximum of 2 infants under the age of 12 months due to having a limited number of cribs available. The designated napping area measures for 38.66 square feet. The toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water is available in each classroom via water dispenser machine. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 11/14/2023 by Glendale Fire Prevention Bureau.

Cribs meet regulatory requirements. Crib area will be separated within the infant classroom using a soundproof barrier with a gate. Sleep logs will be kept in with the Infant classroom. Needs and Services Plans and Sleep Plans will be kept in the Director office.

Licensing staff observed a total of 2 sinks, 2 toilets, and 1 potty chair available for children’s use. These are sufficient to accommodate the requested capacity. Each classroom was observed to contain a changing table. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director’s office.

Facility will provide AM and PM snacks and lunch. Food will be brought into the facility already prepared by a catering company. Licensing staff observed a food warmer. There is a kitchen area located on the premises that is kept off-limit by a locked door. The kitchen area currently includes a refrigerator, microwave, and sink. The kitchen area and food storage areas were observed to be free of rodents and/or vermin.

The facility currently has a fully fenced playground area. Fencing was observed to be at least four feet high. There are 2 designated play areas: Infant yard and Toddler Yard. The total square footage for the designated infant outdoor activity space is 845.44 square feet, which is sufficient to accommodate the requested capacity. The total square footage for the designated toddler activity space is 2,377 square feet. Shade is provided via a shade canopy. There are sufficient outdoor age-appropriate toys, sandbox, tricycles, and play equipment available on the playground. Drinking water is available via portable water jug. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.
For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with the applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed the applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
