On 03/13/2024 at 9:30 am, Licensing Program Analysts (LPAs), Elka Chavez and Crystal Green conducted an announced PreLicense inspection. Upon arrival, LPAs met with Licensee Representative, Anna Lopez, Carlos Tobar, and Reza Khosrowabadi. This facility is currently licensed the facility number is 198018121 which is licensed to serve 26 preschool children ages 3 to 5 years old.The Licensee, St. Anne’s Family Services, is requesting to change the facility program type to an Infant program with a toddler option component and reduce the overall capacity to 16 children; Capacity of 8 Infant in classroom 1 from ages 12 to 24 months and capacity of 8 toddler in classroom 2 from ages 18 to 36 months. Hours of operation will be Monday through Friday from 7:30 AM to 2:00 PM.

All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed the licensee representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Anna Lopez stated that they will be stored in kitchen area in the mini fridge. A fully equipped first aid kit is located next to the kitchen. There is an operational carbon monoxide detector on site. LPAs observed them in each classroom. All required licensing documents were observed posted near the exit doors of each classroom. Children will be signed in and out at the front of the building near their designated classroom.

LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space for the infant program measured 402.27 square feet, which is sufficient to accommodate the requested capacity of 8 infant children. Total indoor activity space for the toddler option program measured 400.71 square feet, which is sufficient to accommodate the requested capacity of 8 toddler children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filter water jug with disposable cups.

LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 12/04/2023.

LPAs observed both classrooms to have a changing table with at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. LPAs observed a total of 7 sinks and 2 toilets available for children’s use. These are sufficient to accommodate the requested overall capacity of 16 children. There is a separate staff restroom equipped with a toilet and a sink located in classroom 2. The isolation area for children who are ill will be the staff office area located next to the kitchen.



Facility will provide breakfast, lunch and PM snack. The kitchen area currently includes a food warmer, microwave, full size refrigerator, mini size refrigerator, dishwasher, and sink. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Hazardous items in kitchen are inaccessible to children via a child safety gate leading to the kitchen area and the cabinets have pads locks.

The facility currently has a fully fenced playground area. Fencing is made of chain link with privacy fence panels which was observed to be at least four feet high. The total square footage for all the outdoor activity space is 2178.82 square feet, which is sufficient to accommodate the requested capacity. At this time, the facility does not have a designated Toddler area. The applicant understand that a waiver will need to be requested allowing the Toddlers to utilize the designated Infant yard using a staggered play schedule. Shade is provided via canopies and fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the playground for children ages 6 to 23 months which is properly anchored. There is adequate cushioning in fall zones of climber provided by artificial turf with padding. Drinking water is available via filtered water jug. LPAs observed all hazardous items on the playgrounds to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Licensee Representative, Anna Lopez, was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Licensee Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

No corrections are needed. A license for 8 infants and 8 toddlers for an overall licensed capacity of 16 children will be granted upon a final file review.
