On 1/02/2025 at 10:50 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Representatives, Lisbeth Perez and Isaias Hernandez. The applicant, Eastmont Community Center, has requested to be licensed to serve 30 children: (6) Infants from ages 0 to 18 months in the Infant Room and (24) Toddlers from ages 18 to 36 months in the Toddler Room. Hours of operation will be Monday to Friday from 7:00am to 5:30pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representatives stated that they will keep it in a locked medicine box located in a cabinet in the Kitchen area. A fully equipped first aid kit is also kept in each classroom near the exists. There is an operational carbon monoxide detector on site located near the Kitchen area. All required licensing documents were observed posted outside of the entrance to the facility. Children will be signed in and out at the entrance of the facility. Staff will escort the children to their designated classroom area.

LPA continued to tour the facility and measured all indoor and outdoor activity space. The total indoor activity space for the Infants measured 261.38 square feet which is sufficient to accommodate the requested capacity of 6 Infants. The total indoor activity space for Toddlers measured 821.06 square feet which can only accommodate a capacity of 23 toddlers. LPA observed the Infant room and the Toddler room to be made separate by a clear plexiglass room divider wall that measured to 2.493 feet tall. LPA informed the applicant representatives of the Title 22 Section 101438.3(b)(2) requirement for Indoor Activity Space for Infants if using moveable walls or partitions shall be at least four feet high; shall be constructed of sound-absorbing material; and shall be designed to minimize the risk of injury to infants.

LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including small tables, chairs, cubbies, 6 cribs, napping cots for infants who can climb out of a crib, bookshelves, and other activity supplies for the children. The toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water will be available to children via water dispenser machine. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 11/27/2024 by LA County Fire Department.

Feeding tables have broad-based legs, wooden seats are in good repair. Cribs meet regulatory requirements. Crib area was observed located in a separate room with walls which will act as a soundproof barrier with a gate. Sleep logs will be kept in with the Infant nap classroom. Needs and Services Plans and Sleep Plans will be kept in the Infant Room.

Licensing staff observed a total of 3 sinks, 2 toilets available for children’s use. These are sufficient to accommodate the requested capacity. There is no changing table located within the designated Infant areas. LPA observed a change table located within the designated Toddler area within arms reach of a sink. Changing table have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the tables was observed located within arm’s reach of a sink. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be located in the room next to the children restroom.

Facility will provide AM and PM snacks and Lunch will be provided by parents. There is a kitchen area located between the Infant crib room and the children restroom that will be kept off-limit by a baby gate. The kitchen area currently includes a refrigerator, microwave, and sink. LPA observed a crockpot located in the kitchen area that will be used for warming infant bottles. The kitchen area and food storage areas were observed to be free of rodents and/or vermin.

The facility currently has a fully fenced playground area that is designated as the Toddler play yard. Fencing was observed to be at least four feet high. The total square footage for the designated Toddler outdoor activity space is 1,656.29 square feet, which can only accommodate 22 children at one time. This facility does not have a separate designated outdoor space for the infant program. Per Lisbeth, a waiver will be requested to allow the infants to use the toddler outdoor space. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure.

Shade is provided via a shade canopy. There are sufficient outdoor age-appropriate toys, sandbox, tricycles, and play equipment available on the playground. Drinking water is available via portable water jug. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.
For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with the applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed the applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed with applicant representatives the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. During this inspection, LPA discussed with the Applicant Representatives options available regarding the outdoor space and the location of the changing table for infants which was observed located in the toddler designated area. Per Applicant Representatives, informed they will submit a waiver letter to the Regional Office. The Applicant Representatives was also informed that all waiver requests will be reviewed by the Regional Office for final approval in order to operate the infant program.

The following corrections are needed prior to the issuance of the license:
  • The Applicant must submit proof of the Infant room movable wall partition to be at least four feet high and made of sound absorbing material.
  • The Applicant must submit and be approved for a waiver regarding the infant changing table located in the toddler designated area.
  • The Applicant must submit and be approved for a waiver regarding the shared use of the designated toddler outdoor space with the infant program.


Applicant Representative, Lisbeth Perez, understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. Once all corrections are received and reviewed, and the final approval is granted for the requesting waivers. A license for (28) children: (6) Infants from ages 0 to 18 months and (22) Toddlers from ages 18 to 36 months will be granted. Exit interview conducted and report was reviewed with the Applicant Representative, Lisbeth Perez.
