1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 4/23/2026 Licensing Program Analyst (LPA) Monica Ruiz conducted an unannounced annual random inspection of the facility noted above. LPA was greeted by Licensee, Jose Romero. LPA presented identification, disclosed the purpose of the inspection to Licensee and provided LIC126- Entrance Checklist. Licensee states that there are currently fourteen (14) children enrolled, there were 11 (eleven) children present during today’s visit, and three (3) children are school age. Also, present was Licensee’s assistant. All individuals present or living in the home have obtained criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home. Regulation regarding visitors was discussed. The children's roster was reviewed and is current. Per Licensee, the facility’s hours of operation are 6:30am - 5:30pm, Monday – Friday. LPA Ruiz informed Licensee that any changes to the facility’s hours of operation, people providing care to day care children, or changes to the home, must be reported to Licensing immediately. Emergency Disaster Plan, License, and Parents’ Rights were observed to be posted on the parent board, which is on the wall, behind the front door. Disaster drill log was available for review during today’s inspection. The last emergency drill documented was conducted on 4/10/2026. This is a one story home which consists of 1 bedroom, 1 bathroom, living room, dining room, kitchen, office, front porch, front yard and main entrance walkway/driveway. FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:

Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Per licensee, areas off limits to children and parents include 1 bedroom, kitchen, and office. According to the licensee, there are no pets on the premises.

All areas identified on the facility sketch were inspected to ensure that they are clean and orderly with ventilation and heating for the safety and comfort of the children. There is a working telephone maintained in the home.

Day care takes place in the living room, dining room, and enclosed front yard. LPA observed age-appropriate toys such as blocks, books, art supplies, dramatic play materials, and sensory toys all free of loose and sharp parts.

Licensee states that children nap in the living room on mats. LPA observed mats and play yards for children and infants to be stored in the living room against the wall. There are cubbies for children in care to store their belongings.

Licensee states that there are no poisons stored in the home. Licensee does understand that poison must be locked. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Personal Rights of children were observed by LPA at time of inspection.

Licensee states that children play outside in the front yard daily. The front porch area is enclosed and there is shade. LPA Ruiz toured the front yard and observed a play structure and slide, and a dramatic play house, all in good condition, free of loose, sharp, or pointed parts.

Trees, grass, shrubs, and plants are maintained. LPA reminded licensee that 100% supervision is required at all times when children are playing outside.

Licensee provides food for children in care. Licensee provides morning snack, lunch, afternoon snack and dinner. Licensee works with Options food program and MAOF resource and referral agency.

Licensee states that they use physical distancing to isolate sick children until their parents' can pick them up.

Per Licensee, there are no weapons or firearms in the home, and none were observed during this inspection. Licensee states there are no pools or bodies of water on the premises, and none were observed on this day.

The valve on the required 2A 10BC fire extinguisher indicates fully charged and has a service tag dated 11/12/2025. Fire extinguishers are to be serviced or replaced annually. Smoke and carbon monoxide detectors were tested at 10:30am and are functioning at this time.
LPA observed First Aid Kit to be stored in the bathroom and was inventoried for necessary supplies.
The following were discussed:
Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee and facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see
PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Licensee, Jose Romero.
