CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced a case- management licensee- initiated inspection to the above facility on 04/09/25 due to licensees are requesting capacity increase. LPA arrived at the facility at 8:40 AM and met with licensees, Nune Sarukhanyan and Karen Durgaryan who guided analyst on a tour of the facility. Also present during this inspection, was Astghik Manasyan, Licensee’s Assistant/cook and Lili Berberyan, licensees' relative. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensees. Per licensees, facility operation hours are Monday to Sunday, 5:00 AM to 11:59 PM. Per licensees, they provide care to children 0-12 years old.

Fire Clearance for Large Family Child Care Home was granted on 03/31/25.

This is a one story home located on the first level. The home consists of 3 bedrooms, 1 & 1/2 restrooms, living room, dining room, kitchen, laundry room, detached garage, balcony (patio), front yard and backyard (fenced). Per licensees, parents enter the home through the side gate which leads to the backyard, balcony and daycare area.

Per licensees the children use the half bathroom in the bedroom, dining room, one bedroom, balcony (patio), and backyard (fenced). Per licensees areas off limits to children and parents include: Living room, two bedrooms, one bathroom in the hallway, kitchen, laundry room, detached garage, front yard, and off-limit areas in the backyard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 8:50 am, the licensees began touring LPA’s through the home starting with the entry way which lead directly to the living room. The living room was inspected and LPA observed sofas, a dining table, and chairs in the living room. There is a fire place in the living room which has been blocked off to prevent access. Next LPA toured the dining room (daycare area) and observed sofas, children tables, chairs and toys in the dining room. LPA observed two child safety gates were installed in the dining room to make the living room and kitchen inaccessible to the children. Next LPA toured the off-limit kitchen and observed child safety latches on the cabinets where licensees stores sharp items and cleaning compounds. Next LPA toured the off-limit laundry room and did not observe any hazards. There is a door in the laundry room which leads to the back balcony (patio). LPA observed the laundry room door was locked during this inspection. Next LPA toured one day care bedroom (main daycare area) and observed 6 children were playing in the daycare room with licensees. LPA observed children toys, tables and chairs and one pack n' play in the daycare bedroom. There is a door in bedroom which leads to the back balcony (patio). There is a door in the daycare bedroom which leads to the off-limit areas. LPA observed the bedroom door was closed during this inspection to make the off-limit areas inaccessible to the children. Next LPA toured the off-limit two bedrooms and off-limit one bathroom in the hallway and did not observe any hazards. Per licensees, no poisons are kept in the home. The required 2A 10BC fire extinguisher was anchored to the wall in the laundry room. It was purchased on 03/31/25 as read by LPA from receipt. Licensees tested the carbon monoxide and smoke detectors in the hallway of bedrooms. It sounded off the alarms and heard by LPA to be functional. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children.

Next LPA toured the back balcony (patio) and backyard (fenced). Per licensees, children use the back balcony (patio) and backyard (fenced) for outdoor play time. The outdoor play area was observed to be fenced. The outdoor playcare ground is covered with artificial grass. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA observed the balcony (patio) fencing is five feet high. LPA observed a gate in the patio which leads to the stairs. Next LPA toured the off-limit garage and did not observe any hazards. The licensees state that supervision is always provided.

Per licensees, they provide food for children in care. Licensees were advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated. Per Licensees, there are no pets, weapons, firearms, pools, spas, hot tubs, fish ponds, or similar bodies of water on the premises.

The licensees were observed to be operating within the licensed capacity and is not exceeding the required limitations during this inspection. During this inspection 6 children were present in the home with licensees and one assistant. All adults present have obtained a criminal record clearance on Licensing Information System (LIS). Per licensees, currently they have one assistant. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The licensees and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensees', Nune Sarukhanyan and Karen Durgaryan Pediatric First Aid and CPR expire on 01/2026. Licensees, Nune Sarukhanyan and Karen Durgaryan completed required mandated reporter training on 01/25/24. There are first aid supplies available on the premises in the daycare room. LPA advised licensees that if a child shows signs of illness he/she/they shall be separated from other children.



At 10:00 a.m., facility roster was reviewed and completed. Per licensees currently there are 12 children enrolled. LPA reviewed 5 children's records, including emergency information from 12 enrolled children, and all 12 reviewed files were completed.

The licensees and her assistant have proof of immunization against influenza, pertussis, and measles. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 03/02/25.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted at the wall by the entrance in the backyard.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. There is telephone service via a cellphone that is used and the cellphone stays at the facility during operation hours. Per licensees, they carry liability insurance in accordance with standard established by Family Child Care statue. Per licensees, if it will be needed they will provide transportation.

INFANT CARE: Licensees state that currently they have two infants enrolled in their daycare. Licensees state that infants sleep in the dining room and one daycare bedroom where they are constantly supervised. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. LPA observed infant nap chart on two infants files. LPA observed LIC 9227 in one infant under one year old file. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Medication: Incidental Medical Services (IMS) policy was discussed. The licensee states that she will provide IMS. Per licensee, there are no children enrolled that require IMS at this time. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA advised the licensee to access forms, regulations and quarterly updates online at: www.ccld.ca.gov.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Licensees, Nune Sarukhanyan and Karen Durgaryan were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



During the exit interview, the Licensees, Nune Sarukhanyan and Karen Durgaryan confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
