On 2/08/2024 at 1:30 pm, Licensing Program Analysts (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Representative, Karine Sahakyan. The Applicant, Smiley Angels Child Care Center Inc., has requested to be licensed for 40 preschoolers ages 2 to 6 years old. Hours of operation will be Monday to Friday from 7am to 6pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA observed the facility to have 2 floors. The main office is located on the second floor accessible via the red classroom. LPA informed Karine Sahakyan that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Karine Sahakyan stated that they will be stored in a locked cabinet located within the Kitchen area. A fully equipped first aid kit is located in within the Kitchen. There is an operational carbon monoxide detector on site located throughout the facility. All required licensing documents were observed posted next to the main entrance to the facility. Children will be signed in and out outside at the main entrance.

LPA continued to tour the facility and measured all indoor and outdoor activity space. This preschool program will have three classrooms (yellow classroom 1, red classroom 2, and green classroom 3) located on the first floor and Learning Enrichment Room located on the second floor. LPA advised the applicant representative, Karine Sahakyan to maintain child safety knob on the door handle that leads to the second floor and the child safety gate at the top of the stairs. Total indoor activity space measured 1081.07 square footage, which can only accommodate 37 children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via water dispenser with disposable cup.

Page 2: LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 5/07/2024 by the Glendale Fire Prevention Bureau.
LPA observed a total of 3 sinks and 3 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 40 children. There is a separate staff restroom equipped with a toilet and a sink located on the second floor. The isolation area for children who are ill will be the staff restroom.

Facility will provide AM/PM snacks. Parents will be providing their child’s lunch. There is a fully functioning kitchen located on the premises which will be used to prepare food on site. The kitchen is located next to the yellow classroom which will be accessible to the children. The kitchen area currently includes refrigerator, sink, stove, oven, food prep countertop. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials.

The facility currently has a fully fenced playground area. Fencing was observed to be at least four feet high. LPA obtained measurements of the designated area which calculated to be 3191.35 square footage which can accommodate 40 children at one time. Shade is provided via canopy. There are sufficient outdoor age-appropriate toys and play equipment available on the main playground area. There is sufficient cushioning in fall zones of the playground. Drinking water will be available via water fountain located on the outdoor playarea. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representative, Karine Sahakyan, was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA advised applicant to keep the document on file regarding completing the lead testing.

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA reviewed with the applicant representative the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The Applicant Representative, Karine Sahakyan, also is an existing licensee for Sahakyan Family Child Care Home 198017114. During this inspection, Karine Sahakyan, has been advised that in order for her to maintain her family childcare license, she must be present in the home 80% of the time.

No corrections are needed. A license for 37 children will be granted upon a final file review and approval of the outdoor waiver request.
