25 | CASE-MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case- management inspection to the above facility on 05/31/2024 due to licensee is requesting to use the detached converted garage for limited activity space. LPA arrived at the facility at 10:40 AM and met with Zinaida Hovhannisyan, Licensee who guided analyst on a tour of the facility. During this inspection Robert Grigoryan, licensee’s husband was also present in the home. Per licensee operation hours will be Monday to Saturday, 7:00 a.m. to 11:00 p.m.. Applicant states she will care for children 0-13 years old.
Fire clearance to use the detached converted garage for limited activity only was granted on 05/23/24.
All areas identified on the facility sketch were inspected. This is a one-story home located on the first level. The home consists of 3 bedrooms, 2 restrooms, living room, dining room, kitchen, detached converted garage which consists of two rooms and one-bathroom, front yard and backyard (fenced). Per licensee, parents enter the home through the driveway gate which leads to the backyard and one daycare room. Per licensee, parent also enter the home through the main entrance which leads to the living room.
Areas that are accessible to children are as follows: Bathroom in the hallway, one bedroom adjacent to the kitchen, living room, dining room, backyard (fenced), and detached converted garage for limited activity only.
Areas off limits based on facility sketch submitted to children and parents include: Two bedrooms, one bathroom in the master bedroom, and front yard.
