On 10/08/2024 at 12:20 a.m., Licensing Program Analyst (LPA) Crystal Green conducted an unannounced Pre-Licensing inspection to add a new School Age Program to an existing Infant and preschool license which will be consolidated into a new single license. Upon arrival, LPA met with Facility Representatives, Brenda Salguero and Samatha Massengale.

This location is licensed as a combination center which serve (48) infant children under (198019482) from ages 0 to 24 months in (6) classrooms: Infant Room 1 and 2, and Toddler Room 1 thru 4; (104) preschool children under (198019483) from ages 2 to 6 years old in (6) classrooms: Preschool Room 1 and 2, Two’s Room 1 thru 3, and the Kindergarten Prep Room.

Applicant, Bright Horizon Children’s Center, LLC., is requesting to add a new school age program with a capacity of 28 children from ages 6 to 12 years and reducing the preschool capacity to 76 children by removing Preschool Room 1 and redesignated as School Age Room 1. Facility operation hours are from Monday to Friday from 8:00 a.m. to 6:00 p.m.

This facility is located on the 2nd floor of the Avision Young building accessible via elevators. When arriving to facility, parents have a key card for the elevator to have access to the second floor. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Licensee Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Licensee Representative stated that they will be located in the Director Office. A fully equipped first aid kit is located throughout the facility.

There is an operational carbon monoxide detector on site located in the Kitchen prep area. All required licensing documents were observed posted near the entrance to the facility. Sign in and out takes place in the classrooms as well as parents checking in on the application called My Bright Day.

LPA continued to tour the facility and measured the indoor and outdoor activity space. Total infant indoor activity space measured 1,784 square footages, which is sufficient to accommodate the existing infant capacity of 48 children. Total preschool indoor activity space measured 1908.39 square footages, which is sufficient to accommodate the requested preschool capacity of 76 preschool children. Total school-age indoor activity space measured 876.25 square footages, which can only accommodate 25 school-age children. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filter water dispenser. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Per Facility Representative, there were no changes to the indoor or outdoor designated infant areas. The infant napping room area was observed to half wall with clear pexi glass window with 8 cribs for non-mobile infants.

This facility has a total of 8 sinks and 4 toilets designated for the infants. The preschoolers have a total of 9 toilets and 12 sinks available for preschool children’s use. The proposed school- age program will have access to restroom located next to the main office area. The restroom contains 1 toilet and 1 sink, which can only accommodate 15 school-age children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the staff restroom.

Facility will provide AM/ PM snacks. Parents will provide their child’s lunch or they will have the option of purchasing a hot lunch via Hero Eats app. The kitchen area is located next to the main office area which currently includes refrigerator, sink, microwave, and food prep counter. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in the kitchen are inaccessible to children via locked cabinet.

The facility currently has two yards: Infant Yard and Preschool Yard. Fencing was observed made of concrete wall which is at least four feet high. The outdoor space was observed to be an indoor / outdoor play area. The indoor area opens into the outdoor area that is separated by a chain fence that is pulled up and down when needed. This facility currently has (2) approved waiver for staggered use of the Infant and Preschool yard. The proposed School- Age Program does not have a separate designated outdoor space.

The Licensee plans to request a waiver to share usage of the designated preschool yard using a staggered play schedule. The total square footage for the preschool yard is 1,908.39, which is sufficient to accommodate 25 children at a time. Shade is provided via fixed building structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is adequate cushioning in fall zones of climber provided by artificial turf and outdoor space pads. Drinking water is available via filter water pitcher and children personal cups. LPA observed all hazardous items on the playground to be inaccessible to children. Licensee Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

This facility will not be providing transportation for the proposed school-age program.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Facility has completed their required lead testing and results were observed posted on the wall near the main entrance.

Licensee Representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.”

This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. The following corrections are needed prior to the issuance of the license:
  • Licensee must submit proof of the designated school age classroom complete with furniture and equipment.
  • Licensee must request and obtain approval for shared use of the designated preschool outdoor space using a staggered schedule.

Upon receipt of the above corrections, this facility will be licensed under a single license for a total capacity of 139 children: (48) infant children from ages 0 to 2 years old in (6) classrooms: Infant Room 1 and 2, Toddler Room 1 thru 4, (76) preschool children from ages 2 to 6 years old in (5) classrooms: Preschool Room 2, Two’s Room 1 thru 3, and kindergarten Prep Room, and (15) school- age children from ages 6 to 12 years old in the School- Age Room 1.
