On 04/30/26, at 8:35 AM, Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced complaint inspection to the above facility delivering above allegation finding. LPA met with Syuzanna Karamyan, licensee who guided analyst on a tour of the facility. During this inspection no children were present in the facility.

During this investigation, LPA Eivazian conducted an interview with licensee, and took pictures from facility outdoor yard and garage.

Per licensee, she has been using the detached converted garage for limited activities per approval that she received from the Licensing Department on 06/21/24. Per licensee, they recently added the swing structure in the yard, and it is safe and children play on it daily and no one gets hurt till this point.

Based on LPA Eivazian observation from swing play structure in the backyard, it was safe and LPA did not observe any hazards regarding play structure.

LPA Eivazian toured the detached converted garage on 04/08/26, and observed children tables, chairs and toys in the detached garage. A fire clearance to use the detached converted garage for limited activities was granted on 06/14/24. LPA Eivazian conducted a case- management inspection on 06/21/24 and approved licensee to use the detached converted garage for limited activity only, children eat either inside the home or outside in the yard and sleep inside the home.

LPA Eivazian sent out a new fire inspection request to the City of Glendale Fire Marshal on 04/03/26 to re- inspect the detached converted garage for children use. On 04/28/26, LPA Eivazian received fire inspection denial that children cannot use the garage for limited activity. On 04/28/26, fire inspection to use the detached garage for children use was denied due to permits missing for garage use as part of the day care. The sprinklers that are installed are rated for garage use, but they are not rated for use when people are in the garage for periods of time.

LPA informed licensee, effective today, she can not use the garage as daycare area and an updated facility sketch was submitted to LPA during this inspection.

An Advisory note was issued on this date in accordance with California Code of Regulations Title 22.
Based on LPA's observations and fire inspection denial, the preponderance of evidence standard has been meet, therefore the above allegation is found to be Substantiated at this time. Title 22 Regulation code 102352(f)(1)-- Definitions. Child Care shall be provided in the providers own home, for periods of less than 24 hours a day, while the parents are away is being noted on the attached advisory note.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
