25 | On 1/15/26, at 8:30AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Annual/Random Inspection to the above facility. Upon arrival, LPA met with Licensee / Hasmik Nazaryan, who assisted wit the visit. LPA disclosed the purpose of the visit. Entrance Checklist – Family Child Care Homes was provided to the Licensee. The licensee has requested a capacity increase for a Large Family Child Care Home for a total capacity of 14 children, ages 0 months to 14 years, which is currently pending. Fire Clearance for Large Family Child Care Home was granted on 12/17/2025. The Licensee guided the LPA on a tour of the facility. Adults residing in or present in the home were discussed, and all have criminal record clearances. Also present was, Licensee’s husband Arman Mkrtchyan. One child was present during today's inspection. The child is the Licensee's own child. Licensee states that there are currently 2 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Saturday, from 7AM - 10PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 12/3/25.
This is a single story home which consists of two bedrooms, two bathrooms, a living room, a kitchen, two side yards, a back yard and a front yard. In addition to the main home, the property includes two separate dwelling units, each with its own address and private entrance. Both units are located behind the main residence and function as independent living spaces. The addresses are 324 W Elk Ave, Unit A and Unit B. According to the Licensee, parents drop off and pick up their children at the main entrance, which leads to the living room. Areas that are accessible to children are as follows: Living room, bathroom 1 and front yard (play area). The home does not have a fireplace. The front yard (play area) has artificial grass with age appropriate toys. Per Licensee, the following areas are off-limits: the kitchen, Bedrooms 1 and 2, Bathroom 2, the two side yards, the backyard, and the two separate dwelling units located behind the main home. All off-limits rooms and areas are made inaccessible through the use of childproof door knobs/locks and child safety gates. The washer/dryer are located inside the kitchen.
