25 | On 06/30/26, at 10:40 AM, Licensing Program Analyst (LPA), Anomeh Eivazian, conducted an unannounced case- management inspection to the above facility. Upon LPA's arrival to the facility LPA met with Tagui Azizian, Nelli Shahumyan, and Nare Chakhmakhchyan, licensee's assistants. During this inspection, Anahit Zakharyan, licensee's assistant arrived to the facility.
Upon LPA's arrival to the facility, licensee, Marine Azizyan was not present in the home. She called LPA Eivazian and informed her that she is in Armenia due to family emergency and she will not be able to be present in the home today. Per licensee, she went to Armenian on 06/25/26 at night. Licensee, did not inform the department that she will be out of country, and during her absence her facility will be open.
Upon LPA's arrival at the facility, at 10:40 AM, LPA observed total of 22 children were present in the home. LPA observed 10 children were in the living room, on the table and doing lessons with Nelli Shahumyan, licensee's assistant, 10 children were in the daycare bedroom with Nare Chakhmakhchyan, licensee's assistant, and Tagui Azizian, assistant was in the kitchen.
Nare Chakhmakchcyan, who stated that she is working at this facility for two weeks did not have criminal record clearance on file. $500 civil penalty was accessed during this inspection.
Facility representative, Anahit Zakaryan was not able to provide LPA a copy of current facility roster.
