32 | Shahumyan, licensee's assistant, and 11 children were in the daycare bedroom with Nare Chakhmakhchyan, licensee's assistant, and Tagui Azizian, assistant was in the kitchen. Also, with LPA's arrival, a guy entered the facility who introduced himself as Gagik Ayvazian who stated that he is coming to the facility for Scratch program.
By 11:40 AM, 10 children were picked up and 13 children were left at the facility.
At 11:00 AM, Anahit Zakharyan, assistant guided analyst on a tour of the facility. During this inspection LPA observed off-limit rooms doors were closed.
During this investigation LPA conducted interviews with three staff, two children, and three parents.
Based on LPA's observation during this inspection 23 children were present in the home with licensee's four assistants. The licensee was observed not to be operating within the licensed capacity and is exceeding the required limitations during this inspection with 8 additional children.
Based on LPA observations on 06/30/26, the preponderance of evidence standard has been meet, therefore the above allegation is found to be Substantiated at this time. Title 22 Regulation code
102416.5 (a)-- Staffing Ratio and Capacity--(a) The capacity specified on the license shall be the maximum number of children for whom care may be provided at any time is being cited on the attached LIC 9099D.
LPA Eivazian informed facility representative, Anahit Zakharyan that this report dated (06/30/26) document 1 Type A citation. Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Eivazian informed the facility representative, Anahit Zakaryan to provide a copy of this licensing report dated 06/30/36 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days.
