25 | Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an announced Case Management – Licensee Initiated inspection at the above-referenced facility on January 7, 2026. The purpose of the visit was due to the licensee’s request for a capacity increase. LPA arrived at the facility at 9:30AM and met with Licensee / Shoghakat Hakobyan, who guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present was, Licensee’s husband / Davit Siradeghyan. There were 6 day care children present during today’s inspection. Licensee states that there are currently 7 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Sunday, from 7AM - 10PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 8/4/25. Fire Clearance for Large Family Child Care Home was granted on 12/17/2025.
This is a 2 story home which consists of a living room, dining room, 3 bedrooms, 2 bathrooms, kitchen, storage room (located next to the kitchen) front yard, back yard and side yard. In addition to the main dwelling unit, the property includes a separate dwelling unit, with its own address and entrance. The separate residence is a detached structure located in the back of the main dwelling (behind the enclosed playground area) at 325 S Madison Way, Unit A, Glendale, CA 91205. A pending application for a Family Child Care Home is currently associated with this address. According to the Licensee, parents drop off and pick up their children at the back yard gate, which serves as an entrance to the playground area. Areas that are accessible to children are as follows: Bedroom 1, bathroom 1 (which is located inside of bedroom 1), living room, dining room, and backyard / playground area. The home has a fireplace located in the dining room, which has been made inoperable and is used for decorative purposes only. The washer/dryer are located inside of bathroom 1. Per Licensee, off-limits areas are as follows: Kitchen, storage room (located next to the kitchen), bathroom 2, bedrooms 2 and 3 (which are located on the second floor), front yard, side yard and the detached structure located in the back of the main dwelling. The off-limits rooms are made inaccessible by child proof door knobs/locks.
