On 2/18/2025 at 10:00 am, Licensing Program Analysts (LPAs), Elka Chavez and Crystal Green conducted an announced Pre-Licensing inspection for an Emergency Relocation due to the recent LA Fire Disaster. The previous facility 191222104 was licensed to operate at 2501 N. Maiden Lane Altadena, CA 91001 as a Preschool Program with a capacity of 60 children. Upon arrival, LPA met with Applicant Representative, Anna Babayan. The applicant, Sahag-Mesrob Armenia Christian Preschool, has requested to be licensed to serve 60 preschoolers from ages 2 to 5 years old in Preschool Rooms 146 (K3 and K4), and 149A (K2). Hours of operation will be Monday to Friday from 9:30 am to 2:30pm. The facility will also offer after school care from 2:30pm to 5:00pm.

This facility is located on the premises with First Church of the Nazarene of Pasadena. There is an existing licensed preschool 191225946 (Sunrise Preschool) located on the premises that was observed to be operating separately from both indoors and outdoors. This childcare center will be located in the Reed Hall building. Per Applicant Representative, there will also be a private elementary school that will be temporary operating within the Reed Hall Building that will be kept separate from the preschoolers both indoors and outdoors. Parents will access the facility from the church parking lot located towards the back of the Reed building.

All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. The facility will not handle medications. LPAs did not observe a first aid kit on site. LPAs did not observe a carbon monoxide detector on site. LPAs did not observe the required licensing documents to be posted. Applicant Representative stated that it will be posted in the hallway. Children will be signed in and out via Brightwheel App.

LPAs continued to tour the facility and measured all indoor and outdoor activity space. The total indoor activity space for Preschoolers measured 2214.35 square feet which can accommodate the requested preschoolers. LPAs did not observe the indoor activity space to be complete with age-appropriate furniture and equipment, including small tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. LPAs observed the carpet located within room 146 and 149A to have dark stains. Per Applicant Representative they will be replacing the carpets and painting the classrooms. LPAs observed the classrooms to not have outlet covers. LPAs observed the blinds located in room 146 to have long cords that are less than 1 foot from the floor. Drinking water will be available to children via bottled water. There are no bodies of water or weapons on the property. Fire clearance has not been granted by Pasadena Fire Department.
Licensing staff observed a total of 3 sinks and 4 toilets available for children’s use. LPAs observed that 3 of 4 toilets were not age appropriate. LPAs observed 1 out of 2 sinks to not made age appropriate. There is a separate staff restroom equipped with a toilet and a sink located in the Reed Building labeled as the Men’s Restroom. The isolation area for children who are ill will be located in the hallway. Per Applicant Representative, the area will be isolated and will contain a cot for ill children.

Facility will provide AM and PM snacks and Lunch. Applicant stated that meals will be delivered ready to serve via a third-party vendor and they will be distributed directly to each classroom.

The facility has access to a fully fenced playground area which will be designated as the Preschool Yard. Fencing was observed to be wrought iron and at least four feet high. The total square footage for the designated Preschool outdoor activity space is 8040.80 square feet, which can accommodate the requested preschool capacity.

LPAs observed the Preschool yard to have sufficient shade in their designated space. LPAs observed the outdoor space to have an age-appropriate climbing structure. LPAs observed the wood chips used a surface cushioning under the climbing structure to be unleveled. LPAs observed sandbox area available for children’s use that was also observed to be unleveled. Drinking water will be available via bottled water. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

· The Applicant must level the sand and wood chips located within the outdoor play area.
· The Applicant must submit proof of making the cords for the blinds located within room 146 inaccessible.
· The Applicant must submit proof outlet covers for areas used by children.

LPAs discussed with the applicant representative the additional pending documents that must be submitted to complete the application process. However, at this time due to the emergency order issue by the governor, the applicant can submit the requested documents within 60 days. Applicant understands that the facility may be issued a provisional license in the event the additional requested documents need time for completion. Once all corrections are received and reviewed, A license will be issues for (60) children: Preschoolers from ages 2 to 5 years will be granted. Exit interview conducted and report was reviewed with the Applicant Representative, Anna Babayan.
