32 | Allegation #1:
Based on four staff interviews, at the time of the incident on 02/23/26, staff#3 and staff#4 were present with 7 children outside at school front yard sand box area. Per staff#3, she was helping another child by the book area in the front yard, staff#4 was in the sand box area. Per staff #3, she did not see the incident, the minute before she saw child#1 was walking on the log, she heard next staff#4 said child#1 are you ok, then staff#3 went to get an ice pack, and brought child#1 to her.
Per staff#4, she was standing in sand area in a way face forward to the sand area, her back was to the office and she was looking at the kids in sand area, in a second she was watching other child on the crate in sand area, did not see how child#1 fell but once child#1 cried, staff#4 went to child#1 and saw a red big bump on child#1’s forehead.
Based on LPA Eivazian observation from video footage of 02/23/26, from 9:00 AM to 9:30 AM : LPA observed child#1 was playing outside in sand area. At the time of incident LPA observed staff#3 and staff#4 were present in the front sand area. Staff#4 was in the sand area and her back was to the office and facing forward to the sand area and tree logs and the staff#3 was holding a child and when child#1’s fell, staff#3 back was toward the log area. LPA observed staff#4 went to the child#1 right away after child#1 fell and hold child#1 and took child#1 to the staff#3.
Based on staff#3 and staff#4 interviews, both staff #3 and staff#4 were present on the school front outdoor playarea, but they did not see child#1’s fell.
Allegation #2:
Based on staff#1, child#1 was dropped off on 02/23/26 at 9:00 AM by dad and incident occurred at 9:17:26 AM based on the facility video footage of the camera. At the time of the incident staff#3 and staff#4 were present on the playground with children, ice was applied, staff#1 assessed child#1 right away to make sure child#1’s eyes were fine, child#1 was not acting disoriented, child#1 was talking and crying. Per staff#1, gave 5-10 minutes, and it was really swollen. Per staff#1, she messaged parent#1 and send picture through procare at 9:33 AM. Parent#1 replied in procare at 9:47 AM, and said she is on her way. Parent#1 arrived at 10:17 AM and picked up child#1.
