On September 3, 2026 at 8:40 am Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced annual inspection at the above facility. LPA was greeted by Licensee, Marisa Beasley who guided LPA on a tour of the facility. Per licensee there are 5 children enrolled, and LPA observed 3 infants and 2 preschool aged children in care. Hours of operation are Monday - Friday from 7:30 am – 5:30 pm.

Family members residing in the home are 1 adult who obtains criminal record clearance and one minor child. This is a single-story home that consists of 3 bedrooms, 1 bathroom, living room, daycare room, kitchen, front yard, backyard (fenced) and detached garage. The facility previously had an above ground swimming pool that has been removed. LPA observed the area where the above ground swimming pool was located and confirmed it has been removed.

Off limit areas for children and parents are 3 bedrooms, kitchen, backyard and detached garage

Areas accessible to children in care are the daycare room, living room, bathroom which is located in the hallway, and front yard.

At 9:10 am licensee guided LPA on a tour of the facility. Per licensee, parents enter through the front door of the home which leads them to the daycare room. Upon entrance LPA observed age-appropriate toys for children in care, children cubbies to store person belongings and a changing table that meets title 22 regulations. LPA then inspected the living room area where LPA observed 2 play pens as well as cots for children in care. The play pens have tight fitted sheets and mattresses. Per licensee all bedding is washed at the facility weekly.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The bathroom in the hallway was then inspected and was clean and orderly. LPA observed a child safety gate between the living room and the kitchen preventing children to have access to the kitchen area. The front yard was then inspected and observed several age appropriate toys and outdoor equipment for children. All areas were observed to be clean, orderly, comfortable, and well ventilated.

There are first aid supplies available. Per licensee, the facility provides all meals for children in care. LPA reminded the licensee that food brought from home must be labeled with the child’s name, stored properly and or refrigerated.

If a child becomes ill, the child is isolated in the living room while the parents are called. A play pen / cot is available if needed.

Licensee's poisons, detergent, cleaning compounds, medications and other items which could pose a danger to child are stored where they are inaccessible to children.

LPA observed a functioning smoke/carbon monoxide detector in the daycare room. LPA observed a 2A-10BC fire extinguisher in the living room area. The valve on the fire extinguisher was pointing to the fully charged green section. Licensee would not locate receipt to show LPA when it was purchased. LPA reminded licensee, receipts should be saved and kept hand for inspection. There is a working telephone on site. Per Title 22 Emergency Disaster Drill must be conducted and documented at least every 6 months. LPA observed the last disaster drill to be conducted and documented on 7/07/26.

Per the licensee, there are no weapons or bodies of water on the premises. During the inspection, LPA did observe 2 small dogs located in the back yard which is inaccessible to children in care. LPA reminded licensee that it is best practice to provide 100% supervision when daycare children are around the pets in the home. Licensee obtains a current Pediatric First Aid and CPR certification which expires on 09/27/2027.

LPA did conduct a children file review and deemed it not complete. 1 out of 5 children were missing LIC 282, 1 out of 5 children were missing LIC 995A, 3 out of 5 children were missing immunizations records, 2 out of 5 children were missing LIC 627, and 3 out of 5 children were missing 15 minute safe sleep logs in their file for LPA to review.

LPA conducted staff file review and deemed it complete.

Licensee has completed mandated reporter training which expires 07/16/2027. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years at www.mandatedreporterca.com

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Notification of Parents' Rights Poster (PUB 394) and Earthquake preparedness Checklist.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
