On August 28, 2026, Licensing Program Analyst (LPA) Monica Ruiz visited the facility noted above for the purpose of conducting an unannounced annual random inspection. LPA met with Licensee, Patricia Chavez Castillo who guided analyst on a tour of the facility. Also present during this inspection were 6 children and Licensee’s assistant. Per licensee, there are 13 children enrolled. Operation hours are Monday – Friday 05:00 a.m. - 06:00 p.m.

Emergency Disaster Plan, License, and Parents’ Rights were observed to be posted on the Parent Board in the living room/main day care area. Disaster drill log was also available during today’s inspection; last disaster drill was conducted on 8/3/26.

The facility is a one-story home which consists of 3 bedrooms, 2 bathrooms, living room, kitchen, front yard, storage room, backyard (fenced), and detached garage.

Licensee states there is one 1 dog on the premises that is kept in an off-limits bedroom. All individuals present or living in the home have obtained criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home.

• Day care areas: living room, 1 bathroom, and back yard.

• Off limit areas: 3 bedrooms, 1 bathroom, kitchen, storage, front yard, and


detached garage.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

**Rooms that are off-limits need to be made inaccessible during operating hours** The applicant does understand that licensing staff may have access to off-limit areas during inspection visits if necessary.
Areas used by children were inspected for safety, comfort, and cleanliness. Tour began in the living room where LPA observed age-appropriate toys, equipment, and materials available for children. LPA observed the required postings on the parent board in the living room. LPA also observed cribs, a play yard and several cots in the living room. LPA then toured the children’s bathroom located at the end of the hallway. LPA observed that the children’s bathroom is in safe and sanitary conditions. There are 3 bedrooms and 1 bathroom that are inaccessible to children.

LPA observed that all bedrooms are inaccessible to children and have child safety doorknob covers or are locked. Per Licensee, the kitchen is off-limits to children. LPA observed a child proof gate between the kitchen and living room. Licensee stores all knives in a high cabinet, inaccessible to children.

Per Licensee, there are no poisons on the premises. Licensee was advised that any poisons must be locked with a key or combination lock.

LPA observed (1) fire extinguisher in the living room. The 2A10BC fire extinguisher was fully charged and was serviced on 7/14/26, per the attached receipt.

Per Licensee, she provides 3 meals and snack to all children in care. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with the child’s name and properly stored or refrigerated.

Licensee tested the smoke and carbon monoxide detectors at 11:15 am and they were observed to be in operable condition.

At 11:30am LPA toured the back yard where the detached garage is located. Per Licensee, children use the backyard for outdoor play. LPA observed a barricade between the children’s yard and the garage to make the garage area inaccessible to children.

LPA observed age-appropriate play equipment for children in care such as a dramatic play house, water activity table, and a climbing structure. LPA observed a padded turf material under climbing equipment to absorb any potential falls.

LPA advised the Licensee to check with the American Standard Testing Materials for Playground Guidelines and/or to use the manufactures recommendations for the playground equipment in efforts to be more in line with safety guidelines. Licensee understands that children should always be supervised while playing in the outdoor


play area. Telephone service is via a cellphone. LPA also observed there to be a camera/doorbell by the front entrance.

Per Licensee there are no firearms, no weapons, and no bodies of water on the premises and LPA did not observe any of the above mentioned during this inspection.

LPA reviewed 6 children’s records and found them to be complete. LPA reviewed staff files and required forms were discussed.

Licensee has current Pediatric First Aid and CPR. During this visit, Licensee called the CPR/First Aid instructor and requested her they send her CPR card via email, LPA heard instructor confirm that Licensee completed course and will send the certificates this evening.

Proof of immunization against tuberculosis, pertussis, and MMR was readily available during today’s inspection. Licensee and assistant have taken Mandated Reporter and CPR/First Aid Training.

The following were discussed:
Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. page 4

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. License states that they do not provide IMS. For IMS information see PIN 22-02-CCP.
