On 4/14/2026, at 10AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. The purpose of the visit was to ensure that the health, safety, personal rights, licensing conditions and limitations are as required by Title 22 Regulations. Upon arrival, the LPA met with Licensee / Sophia Rowland, who assisted with the visit. LPA disclosed the purpose of the visit. The Licensee has requested a capacity increase for a Large Family Child Care Home for a total capacity of 14 children, ages 2 years to 5 years, which is currently pending. The Los Angeles Fire Department conducted a visit on 03/19/2026 and approved a capacity increase to a maximum of 14 children. Adults residing in or present in the home were discussed, and all have criminal record clearances. Individuals present at the time of the inspection included the Licensee’s assistants, Samantha Choy and Daisy Garcia and Licensee's husband, Edison Mellor Goldman. There were 4 day care children present during today’s inspection. Licensee states that there are currently 10 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Friday, from 9AM - 3PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 4/13/26.

The facility is a two story home consisting of 1 bedroom, home office, 1 bathroom, playroom, living room, dining area, kitchen, front/side patio and in ground swimming pool in the back yard, which is fenced. In addition to the home, the property includes a separate dwelling unit, with its own address and entrance. The main residence is a detached structure located in front of the home at 923 S Stanley Ave, Los Angeles, CA 90036, and functions as a separate living space.



Areas accessible to children will be the entire first floor, which consist of the living room, dining area, kitchen, bathroom and playroom. Children will also have access to the front/side patio area which is fenced for outdoor play.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Areas inaccessible to children in care include the entire second floor which includes 1 bedroom and home office, and the swimming pool that is located in the backyard. LPA observed a child safety gate at the bottom of the staircase making the second floor inaccessible. LPA observed the swimming pool to be fully fenced with a mesh gate that is 60 inches tall and is ASTM approved.

According to the Licensee, parents drop off and pick up their children by using the driveway, entering through the gate, and walking them to the Licensee’s main door. The Licensee stated that the gate is always locked, and staff members open it to allow parents to enter. Upon entrance to the property, LPA observed the drive way to be fenced with a ring camera. When entering the home, LPA observed the kitchen area. The kitchen area was clean and orderly. All kitchen cabinets obtained child safety locks making them inaccessible. Kitchen knives were also stored in an area that is inaccessible to children. There is also a closet that is right next to the kitchen area that is locked. In this closet applicant stores cleaning solutions and first aid kit. The First Aid kit was inventoried for necessary supplies. Next to the closet, LPA observed the staircase that leads to the second floor. LPA observed a child safety gate at the bottom of the staircase, making the second floor inaccessible to children in care. Then the dining and living room area was inspected and observed a dining table with chairs, a couch and age appropriate toys for children in care. LPA then toured the playroom which is next to the bathroom and observed a mini couch, additional toys for children in care and children cubbies to store personal items. The playroom also has a storage area where children’s napping equipment in the form of mats were observed. The playroom is also used for napping.



All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. There is a working telephone maintained in the home. LPA observed age appropriate toys, free of loose and sharp parts. LPA observed electrical cover outlets all through the home. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. The Licensee states that there are no poisons in the home. The Licensee understands that poison must be locked with a key or combination lock. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Per the Licensee, there is a pet dog that is kept away from the children and remains on the second floor. Isolation area for sick children is in the living room. The Licensee provides snacks, and parents provide lunch for their children. Food preparation areas were toured for safety, cleanliness and proper equipment. The facility is nor equipped with surveillance cameras. Per Licensee, there are no weapons, firearms in the home.

LPA observed a fire extinguisher type 2A-10BC in the kitchen area with the purchase date of 8/4/2025. Smoke and carbon monoxide detectors were tested and observed to be in operable condition. The Licensee has current Pediatric First Aid and CPR. Proof of immunization against influenza, pertussis, and measles was readily available during today’s inspection. The Mandated Reporter Training for the Licensee is current.

LPA was then guided to the outdoor area. LPA inspected the front patio area and observed multiple age-appropriate toys for children in care as well as an outdoor portable sink. Then the Licensee guided the LPA to the side patio area where LPA observed additional age appropriate toys for children in care. LPA also observed a locked divider gate at the end of the side patio. Beyond the divider gate is the swimming pool area. The Licensee unlocked the divider gate and lead LPA to the swimming pool area. The swimming pool is surrounded by a mesh gate that is at least 60 inches tall, opens away from the swimming pool, is self-closing with a self-latch, and lockable device. The mesh gate did have vertical clearance from the bottom of the enclosure to the ground and is no more than 2 inches. There are no gaps in the enclosure that could allow for a sphere of 4 inches or larger to pass through. The outside surface is free of any physical characteristics that could be used as a handhold or foothold for a child to climb over. LPA also inspected the swimming pool alarm and safety equipment. The pool alarm must sound upon detecting entrance into the water. The Licensee tested the pool alarm and was functioning. As the Licensee placed the life ring in the water and created a wave, the pool alarm started the beep. The pool alarm shall be left on and in working condition and audible during the facility’s operating hours while the swimming pool is not in use. LPA observed and inspected the safety equipment that must be visible from the swimming pool and available for immediate use. The life ring with a diameter of at least 17 inches with a United States Coast Guard approval label and a rescue pole with a body hook that is at least 12 feet long but must be long enough to effectively rescue a child at the facility’s swimming pool. LPA also observed the applicant’s swimming pool safety log. LPA reminded applicant that the swimming pool area and safety material shall be inspected daily and available for the department to observe upon request.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Because the Applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC 9151) that the Applicant confirms was provided to the property owner/landlord. The Applicant has also obtained a signed Property Owner/Landlord Consent form (LIC 9149).

Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed and issued the LIC 311D - Forms/Records to Keep in Your Family Child Care Home. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Licensee was advised how to access forms and Regulations online at www.ccld.ca.gov. Licensee was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

There were no deficiencies issued during today's visit (4/14/26).
