13 | Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced site inspection to present the findings of the above complaint allegation. Licensing staff met with Director Carina Hu, to whom the reason for the visit was explained. Director provided LPA on a tour of the facility. Present during the inspection was 25 children and 8 staff.
During the investigation licensing staff conducted interviews with director and 6 staff. Licensing staff also obtained several documents related to the complaint allegation, including but not limited to, a copy of the Facility Roster, children’s roster, emergency contacts, daily attendance logs, staff contacts and copies of transcripts, parent handbook, employee handbook, school app messages, copy of internal incident reports completed by teachers, and email follow up with parent and school. Infant sleep check logs and infant needs and service plans were reviewed for completeness. Staff files were also reviewed during this investigation.

7 | Per director, center has implemented teacher break report how many children they are leaving behind and with which teachers. Teachers are being trained on small group work to work with a group of 6 children and leaving other co teacher with less children. Open flow with lower yard, teacher in lower yard is the one resposible for counting moving groups. Teachers are being trained as facilitators, to help with potty, take pictures, and find activities for children to do. Procedures have been implemented. Training notes were provided to LPA during visit. |

7 | Per director, she is currently filling in the nap room and providing nap supervision. Per director, 6 toddlers are moving up to next class reducing infant numbers. Teacher breaks are provided by director in stagger shifts so there are always 3 teachers on site for infants. If director is out, fully qualified prek teacher will sub in to break infant staff. Updated break schedules were provided to LPA S Li during visit. |
