On Friday, February 20, 2026, LPA Staicy Perry arrived at the facility to conduct a pre-licensing inspection to inspect and evaluate the facility for a Large Family Child Care Home (14) relocation. Applicant is relocating Caudillo Family Child Care (#198021144) with address of 2629 Pomeroy Ave Los Angeles CA 90033 to a new location at 2740 Fairmount St. Los Angeles CA 90033. Applicant, Brenda Elizabeth Caudillo, who guided LPA on a tour of the facility. The applicant is requesting a relocation on her large family child care home license. Per applicant, operation hours will be Monday to Friday, 6:00 a.m. to 6:00 p.m. Brenda Elizabeth Caudillo, Applicant states that they will care for children from birth - 12 years of age. Pre-licensing Entrance Checklist (LIC 9280) was provided to applicant. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC 811) and attached to this report. Fire clearance was granted on 2/13/2026.

All areas identified on the facility sketch were inspected. This is a house, which consists of 3 bedrooms, 2 bathrooms, kitchen, 1 living room, laundry room, backyard and a front yard.

Off-limit areas:1 bathroom in bedroom #3, Bedroom #3, backyard and laundry room.

On- limit areas: 2 bedrooms, 1 bathroom, kitchen, living room and front yard.

**Rooms that are off-limits need to be made inaccessible during operating hours**

Per applicant, the front yard will be used as the designated outdoor play area for children in care. The backyard is off-limits and has been made inaccessible to children. The licensee confirmed that outdoor play will only occur in the approved front yard area under supervision. LPA observed the front yard to be gated.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Brenda Elizabeth Caudillo, Applicant began touring LPA Perry through the home starting with the entry way. Per Brenda Elizabeth Caudillo, Applicant parents will enter home through the driveway to drop off the children. The parents will then enter the door which leads directly to the living room. Next LPA toured the living room, where parent board was observed with postings, as well children’s toys, television hanging on the wall and a diaper changing area for infants in care. Next LPA Perry toured bedroom #1 and observed cribs and additional mats for older children were in the bedroom closet. LPA Perry observed and toured bedroom #2 and observed toys to be age appropriate as well as a queen size bed and 2 cribs. LPA toured bedroom #3, where licensee will use and keep off limits to the children, a closet in the bedroom was observed with applicants’ personal belongings. LPA toured bedroom #3, where licensee will keep off limits to the children. LPA observed key locks on the bedrooms that are kept off limits to the children. LPA toured the bathrooms and observed shampoos / conditioners stored up high inaccessible to the children. LPA inspected cabinets and drawers in the bathroom and did not observe any hazards and did observe child locks on all drawers. In the kitchen, LPA observed sharp items stored made inaccessible to children located in a high cabinet making it inaccessible to the children. Brenda Elizabeth Caudillo, Applicant, she keeps cleaning supplies in the bottom of the sink making it inaccessible to the children via a child safety lock. LPA observed medication stored in an off-limit area (bedroom #3) inaccessible to the children. LPA observed 2A-10BC required fire extinguisher in the kitchen with the purchase date 12/29/2025. LPA observed smoke and carbon monoxide detectors in every room in the home. LPA observed a mini split AC and heating. Next, LPA Perry observed the laundry room that is currently being used as storage. Per Brenda Elizabeth Caudillo, Applicant, children will not have access to the laundry area as the laundry room area is located in bedroom #3 and is inaccessible via key lock making it inaccessible for the children.

There were toys available for children. There is a telephone service via a cellphone that is used at the facility during operation hours. LPA advised applicant that if a child shows signs of illness, he/she/they shall be separated from other children. Per applicant the bedroom #2 that has a bed will be used as an isolation area until an authorized person picks up.

Per applicant no one smokes in the home. There is first aid kit located in the kitchen and was observed to be fully equipped.

The applicant states that supervision will be always provided. The applicant states that she will provide food for children in care lunch, PM snack and dinner. The applicant was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated. Per applicant, there are no pets, weapons, firearms or bodies of water on the premises. Per Brenda Elizabeth Caudillo, Applicant she is working with Early Head Start from MAOF foundation.

The applicant does have proof of Health and Safety training dated 10/7/2022, Pediatric First Aid and CPR dated 5/21/2024, proof of immunization against influenza, pertussis, and measles. Applicants completed required mandated reporter which expires on 8/6/26. LPA also obserevd CCLD Orientation on file dated 10/25/2022. LPA did advise licensee that Mandated reporter training must be completed every 2 years. www.mandatedreporterca.com . LPA observed the Property Owner Notification forms (LIC9151) and obtained a signed Property Owner Consent form (LIC 9149) which is on file.

The following was discussed with the applicant:

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

-In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current adult/infant CPR & Pediatric First Aid certification, and a valid criminal record clearance associated to the facility license.

-A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility.

-Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the License shall be terminated.

-The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary and smoke and carbon monoxide detectors should check, and batteries replaced as needed.

-Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home.

-Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. (Use LIC624B for written report)

-Fire and safety drills must be performed every six months and documented for review by the Department.

-Smoking is prohibited in a family childcare home.

-Children and Staff records must be maintained and updated as needed and must be available for review by the Department.

-Dog(s) and or pets are recommended to be isolated from children in care.

-No smoking, No infant walkers, No Johnny jumpers, No saucer chairs, No trampolines and any other item that falls into that category are not permitted in the facility. -All adults living and working in the home shall be made of aware of the Departments right to inspection authority, which includes but not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.

- Applicant shall reveal each facility license number in all advertisements, publications or announcements with the intent to attract clients.

- Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are required to be posted.

Applicant states that she will care for infants. The applicant states that infants in the bedroom with open doors will be constantly supervised. Appropriate sleeping arrangements, play yard or cribs will be available once an infant being enrolled. LPA informed one crib for each infant in care will be needed. Cribs or play yard shall not hinder the entrance or exit from the sleeping space, mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Cribs and play yards shall be free of loose articles and objects. No objects shall be hanging above or attached to the side of the crib. LPAs informed applicant that infants cannot be swaddled while in care. LPAs advised the applicant that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked every 15 minutes, and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Applicant. LPA provided the Applicant with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Family Child Care Homes On this date, 2/20/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant Brenda Elizabeth Caudillo was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California

LPA reviewed with applicant Brenda Elizabeth Caudillo the LIC 311D, Forms/Records to Keep in Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance checklist was provided to the applicant.

The applicant’s signature on this report acknowledges that they have signed the Application for a Family Child Care Home License (LIC 279) under the penalty of perjury that the statements on the application and any attachments are correct.

The application will be submitted for final review to the department. No corrections needed. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license.

An exit interview was conducted with applicant,Brenda Elizabeth Caudillo. Appeal rights were reviewed and provided. A Notice of Site Visit was issued.
