PRELICENSING INSPECTION CONDUCTED IN ARMENIAN

Licensing Program Analyst (LPA) Shushanik Safaryan conducted an announced pre-licensing inspection to the above facility on 05/06/2026. Upon arrival , at 08:30 am LPA met with Seda Ghazaryan, applicant who guided analyst on a tour of the facility. Pre-licensing Entrance Checklist (LIC9280) was provided to the applicant. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per applicant operation hours will be Monday to Saturday, 8:30a.m. to 11:00 p.m. Applicant states she will care for children 0-13 years old.

Licensee was previously licensed with facility number 198021762 at 1125 N. Stanley Ave, Glendale, Ca 91206.

All areas identified on the facility sketch were inspected. This is a one-story home that consists of 3 bedrooms, 3 bathrooms, living room, dining room , office space, kitchen , laundry room, front yard, back yard and detached garage. There is a fireplace in the living room which has been blocked off to prevent access to children.


Areas that are accessible to children are as follows: Living room, dining room, office space, kitchen, bathroom in the hallway, bedroom #1 adjacent to the bathroom, laundry room, back yard (fenced).

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Areas off limits based on facility sketch submitted to children and parents include: Two bedrooms, two bathrooms in the bedroom, front yard , detached garage and side yard where AC unit is located.
**Rooms that are off-limits need to be made inaccessible during operating hours**

At 9:00 am, the applicant began touring LPA through the home starting with the entry way which led directly to the Living room (day care room) .The Living room was inspected, and electrical outlets were observed to be covered. LPA observed fireplace in the living room which was screened and secured inaccessible to the children. LPA observed shelves next to the fireplace built into the wall and have child safety latches. Next LPA toured the dining area. Per applicant dining area will have a carpet and age-appropriate toys for children. Children will use office space. LPA observed office space separated from dining area with the child safety gate. Per applicant office space will be used for activities and small table and chairs will be placed. Office space was separated from the kitchen with child safety gate. Per applicant, children will pass through the kitchen to exit to laundry room and outdoor play area. LPA observed child safety latch on the cabinet under the sink where cleaning supplies are stored and sharp items inaccessible to the children in the upper cabinet. Per applicant , no medications will be stored in the kitchen. Next LPA toured the bathroom in the hallway accessible to the children. In the bathroom, LPA observed child safety latches on two cabinets under the sink. Per applicant only soap and paper will be stored in the children’s bathroom. Next LPA toured the bedroom that will be used for day care. The bedroom will be used for napping and cots will be placed for children enrolled. LPA observed storage area with the child safety latch, per applicant it will be used for day care storage. LPA observed empty cabinets in hallway, per applicant day care supplies will be stored. Next LPA toured the off-limit bedrooms and bathrooms and observed family personal belongings. LPA observed chain lock on the off-limit bedrooms. Children will pass through the small area in the kitchen to the laundry room. LPA observed laundry machines stored in the storage that have a child safety latch to be inaccessible. LPA observed cabinet in the laundry room has a child safety latch where laundry detergents and additional cleaning supplies will be stored.
LPA observed the required 2A 10BC fire extinguisher in the laundry room .It was purchased on 05/05/2026 as read by LPA from the receipt. LPA observed carbon monoxide in the kitchen and smoke alarm in the hallway area. Applicant tested the carbon monoxide and smoke detectors .It sounded off the alarms and heard by LPA to be functional.

Per applicant, children will use the back yard for outdoor play time . LPA observed outdoor play area was fenced and gated. LPA observed AC unit was separated by the gate on the side yard. Next, LPA observed outdoor grill which was covered by wooden panels and inaccessible to the children. LPA toured the detached garage, per applicant detached garage is off limit area and always will be locked with key lock. LPA observed children will have access to the carpet area and grass area. Outdoor area has toys and canopy that provides shade for children. LPA observed side gate that exits to the driveway has locks accessible to the children. Per applicant, children will not be left alone outside and will be supervised all the time.

The applicant states that she will provide food for children in care. Applicant was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated.
Per applicant, at 10:00 am, there are no pets, weapons, firearms or bodies of water on the premises. There is telephone service via cellphone that is used at the facility during operation hours. LPA advised applicant that if a child shows signs of illness, he/she/they shall be separated from other children. Per applicant no one smokes in the home. There is first aid kit located in the kitchen cabinet.

The applicant does have proof of Health and Safety Training issued on 10/12/2024. Applicant has proof of Pediatric CPR and First Aid training that expires on 10/2026. The applicant has proof of immunization against influenza, pertussis, and measles.
Applicant completed required mandated reporter training on 06/19/2025. Mandated reporter training must be completed every 2 years. www.mandatedreporterca.com

Because the applicant leases the home proof of landlord notification is required. The LPA observed the Property Owner Notification form (LIC9151) that the applicant confirmed was provided to the property owner. The applicant obtained a signed Property Owner Consent form (LIC 9149).

The following was discussed with the applicant:
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

-In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current adult/infant CPR & Pediatric First Aid certification and a valid criminal record clearance associated to the facility license.


-A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility.
-Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the License shall be terminated.
-The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary and smoke and carbon monoxide detectors should check, and batteries replaced as needed.
-Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home.
-Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. (Use LIC624B for written report)
-Fire and safety drills must be performed every six months and documented for review by the Department.
-Smoking is prohibited in a family childcare home.
-Children and Staff records must be maintained and updated as needed and must be available for review by the Department.
-Dog(s) and or pets are recommended to be isolated from children in care.
-No smoking, No infant walkers, No Johnny jumpers, No saucer chairs, No trampolines and any other item that falls into that category are not permitted in the facility. -All adults living and working in the home shall be made of aware of the Departments right to inspection authority, which includes but not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.

- Applicant shall reveal each facility license number in all advertisements, publications or announcements with the intent to attract clients.

- Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are required to be posted.

INFANT CARE: Applicant states that she will care for infants. Applicant states that infants will sleep in the bedroom and will be constantly supervised. Appropriate sleeping arrangements play yard, and cribs will be available once an infant being enrolled. LPA informed one crib for each infant in care will be needed. Cribs or play yards shall not hinder the entrance or exit from the sleeping space, mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Cribs and play yards shall be free of loose articles and objects. No objects shall be hanging above or attached to the side of the crib. LPA informed Applicant infants cannot be swaddled while in care. LPA advised the Applicant that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Applicant. LPA provided the Applicant with a copy of A Child Care Provider’s Guide to Safe Sleep, by American Academy of Pediatrics. LPA also consulted and explained Child Abuse Reporting, Never Shake a Baby, and Safe Sleeping practices.

SAFE SLEEP: LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

MyChildCarePlan.org--Child Care Centers and Family Child Care Home Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep in Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance checklist was provided to the applicant.
