13 | Licensing Program Analyst (LPA) Ana Soto conducted subsequent complaint tele-visit to provide findings and decision for the allegation listed above . Due to the situation surrounding the Coronavirus Disease 2019 (COVID-19), and to implement mitigation measures, today’s subsequent visit was conducted telephonically with Ella Naygas, facility administrator.
Based on the LPA's investigation, the investigation revealed the following: Facility staff do not keep facility clean. LPA Soto on 05/21/20 @ around 11:00am interviewed administrator, Ella Naygas, Assistant Administrator Clifford Johnson, and Clients #1 - #9, they all stated that the facility is always clean. The clients rooms and restrooms, common areas such as: rest rooms, tv room, living room, dining room, and etc. are clean and well maintained. LPA Soto @ around 11:30am on 05/21/20, inspected the entire facility (clients rooms, patio, rest rooms, living room, dining room, kitchen, storage area, and administrator office.)
