On 09/30/2025 Licensing Program Analyst (LPA) Troy Watson conducted a subsequent unannounced complaint visit to the facilty listed above and explained the reason for the visit was to investigate the allegations listed above. LPA Watson was granted entry into the facility.

The investigation consisted of the following:

On 09/03/2025 LPA Watson Interviewed Staff #1 - Staff #2.On 09/04/2025 LPA Watson interviewed Staff #3 (S3) and requested obtained and reviewed the following:Personnel Report,Client Roster, Special Incident Report, Behavioral Report, Medical Administration Records (MAR's), Superior Court Documentation Centrally Stored Medication Destruction Record (CSMDR). The investigation revealed the following:

Allegation: Staff does not ensure sharp knives are made inaccessible to clients.

On 09/03/2025 LPA Watson conducted an interview with Client #2 & Staff #1-Staff #3 (S1-S3) about the allegation listed above. One out of 2 of clients interviewed denied the above allegation. An attempt to interview Client #2 was made but C2 was not present at the time of the interview. On 09/03/2025 LPA Watson interviewed Staff#1-Staff#3 (S1-S3). Out of those interviewed 3 out of 3 Staff denied the above allegation.On 09/30/2025 LPA Watson conducted an interview with Client #1 (C1) about the allegation listed above. Out of those interviewed 1 out of 2 clients denied the allegation. LPA Watson noted during the interview that no sharp knives were kept or located in the facility. Based on the information gathered, interviews conducted, and review of records LPA Watson found no evidence to support the allegation mentioned above. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation, did or did not occur, therefore the allegation is Unsubstantiated.

Allegation: Staff did not prevent clients from engaging in physical altercations with other adults.

On 09/03/2025 LPA Watson conducted an interview with Client #2 (C2) about the allegation listed above. Out of those interviewed 1 out of 2 of the clients interviewed denied the above allegation. An attempt to interview Client #2 was made but C2 was not present at the time of the interview. On 09/03/2025 LPA Watson interviewed Staff#1- Staff#3 (S1-S3). Out of those interviewed 3 out of 3 Staff denied the above allegation.On 09/30/2025 LPA Watson conducted an interview with Client #1 (C1) about the allegation listed above. Out of those interviewed 1 out of 2 clients denied the allegation. Out of those interviewed 2 out of 2 clients denied the allegation. LPA Watson conducted interviews with staff and found that law enforcement was immediately called after each altercation occurred at and outside the facility. Based on the information gathered, interviews conducted, and review of records LPA Watson found no evidence to support the allegation mentioned above. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation, did or did not occur, therefore the allegation is Unsubstantiated.

Allegation: Staff did not contact emergency services in a timely manner.

On 09/03/2025 LPA Watson conducted an interview with Client #2 (2) about the allegation listed above. Out of those interviewed 1 out of 2 clients interviewed denied the above allegation. An attempt to interview Client# 2 was made but C2 was not present at the time of the interview. On 09/03/2025 LPA Watson interviewed Staff#1- Staff#3 (S1-S3). Out of those interviewed 3 out of 3 Staff denied the above allegation. On 09/30/2025 LPA Watson conducted an interview with Client #1 (C1) about the allegation listed above. Out of those interviewed 2 out of 2 clients denied the allegation. LPA Watson requested, obtained and reviewed court documents obtained from the administrator that showed law enforcement was immediately called in a timely manner after each incident. Based on the information gathered, interviews conducted, and review of records LPA Watson found no evidence to support the allegation mentioned above. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation, did or did not occur, therefore the allegation is Unsubstantiated.

An exit interview was conducted with the Christopher Aaron and a copy of this report was given.
