**This report serves as an amendment to clarify findings. It does not supersede the complaint investigation findings reflected on the report created 10/01/2025. **

On 09/30/2025 Licensing Program Analyst (LPA) Troy Watson conducted a subsequent unannounced complaint visit to the facility listed above and explained the reason for the visit is to deliver findings. LPA Watson was granted entry into the facility.

The investigation consisted of the following:On 09/03/2025 LPA Watson interviewed Staff #1–Staff #2. On 09/04/2025 LPA Watson interviewed Staff #3 (S3) and requested, obtained, and reviewed the following: Personnel Report, Client Roster, Special Incident Report, Behavioral Report, Medical Administration Records (MARs), Superior Court Documentation, and Centrally Stored Medication Destruction Record (CSMDR).

The investigation revealed the following:
Allegation: Staff does not ensure sharp knives are made inaccessible to clients.
It is alleged that staff did not ensure sharp knives were made inaccessible to clients, resulting in a client obtaining a knife from the kitchen and chasing a neighbor with it.
On 04/13/2026 the Department conducted an interview with the Chief Executive Officer (CEO), Christopher Aaron (S1). S1 was asked if sharp objects/knives were kept locked away and inaccessible to clients in the facility, and S1 responded yes. The Department also asked S1 where C1 obtained the knife, and he stated that C1 told him he obtained the knife from a friend in the community.
On 09/03/2025 the Department interviewed Staff #1–#3 (S1–S3). Out of those interviewed, 3 out of 3 staff denied the allegation. On 09/03/2025 and 9/30/2025 the Department conducted interviews with Client #1-Client#2(C1 and C2). Out of those interviewed, 2 out of 2 clients denied the allegation.
On 09/02/2025 the Department conducted an interview with Witness #1 (W1). W1 said he was standing in front of the gate on the sidewalk right in front of the property when C1 came outside the facility with a knife. W1 also said the staff were shouting at C1 and telling him to come back to the house. W1 further stated that C1 went back into the house but came out again with a broomstick and then was called back in again and came out with a mop handle.
The Department conducted a tour with the Chief Executive Officer (CEO), S1, and inspected the kitchen for unlocked knives. The Department observed that the area where sharps were kept was locked and inaccessible to clients.
Based on the information gathered, interviews conducted, and records reviewed, the Department found no evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur; therefore, the allegation is Unsubstantiated.
Allegation: Staff does not prevent clients from engaging in physical altercations with other adults.
It is being alleged that staff did not prevent clients from engaging in physical altercations with other adults, as a client reportedly assaulted a neighbor’s wife and later chased the reporting party multiple times with various objects.

On 04/13/2026 LPA Watson conducted an interview with CEO Christopher Aaron staff#1 (S1). S1 stated that an incident did occur, but that the initial incident occurred around the corner in front of the neighbor’s house. S1 reported that the neighbors later showed staff a Ring camera video that captured how the incident began and he did not see any physical altercations on the video between his client and the other adult. Later that same day, a verbal altercation also occurred, but C1 was in the house and W1 was outside of the property gates.

On 09/02/2025 the Department conducted an interview with Witness #1 (W1). W1 confirmed that during incident at facility, C1 was in facility and on porch, but that W1 was outside of the property gate.
On 09/03/2025 the Department conducted interviews with Staff #1–#3 (S1–S3). Out of those interviewed, 3 out of 3 staff denied the allegation. On 09/03/2025 and 09/30/2025 the Department conducted interviews Client #1 – Client 2(C1-C2) regarding the above allegation. Out of those interviewed, 2 out of 2 clients denied the allegation.
Based on the information gathered, interviews conducted, and records reviewed, LPA Watson found no evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is Unsubstantiated.

Allegation: Staff did not contact 911 emergency services in a timely manner.

It is being alleged that staff did not contact law enforcement in a timely manner, as staff reportedly did not contact law enforcement while a client was chasing a neighbor with a knife and other objects.
On 04/13/2026, the Department conducted an interview with Chief Executive Officer (CEO) Christopher Aaron (S1). The Department asked S1 if the facility ever contacted emergency services (911) for the client–neighbor altercation. S1 said the facility did not contact 911 because the incident was handled by staff before anything could potentially happen. S1 also said that the incident was caused by the neighbors who came over and stood in front of the facility yelling and making threats toward C1. S1 also said that the neighbor called the police after the incident was over, and by the time he arrived there, law enforcement was already on the scene.

On 09/03/2025 and 09/30/2025 the Department conducted interviews Client #1 – Client 2(C1-C2) regarding the above allegation. Out of those interviewed, 2 out of 2 clients denied the allegation. On 09/03/2025, the Department interviewed S1–S3. Out of those interviewed, 3 out of 3 staff denied the allegation.

On 4/15/2026, at approximately 3:44pm, the Department obtained and reviewed the Los Angeles County Sheriff’s Department Incident Report dated 8/6/2025, which showed that the alleged incident started at a different address away from the facility, on a different street.
Based on the information gathered, interviews conducted, and records reviewed, LPA Watson found no evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is Unsubstantiated.

An exit interview was conducted with the Vice President Christopher Aaron, and a copy of this report was provided.
