25 | On 09/24/25, at 9:44am, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced visit to Ivy Homes. LPA met with Oluwarotimi Johnson, Direct Support Professional (DSP), and the purpose of today’s visit was explained. The facility is licensed to operate for (4) ambulatory, (0) non-ambulatory, adults ages 18 through 59. The clients are South Central Los Angeles Regional Center clients. None of the clients have Restricted Health Care Conditions and none utilize postural supports or protective devices. The facilities’ annual fees are current.
The facility is a two-story home located in a residential neighborhood. The property consists of the following: 4 client bedrooms, 2 bathrooms, office, living room, kitchen, dining room, attached garage which houses the washer and dryer and outdoor shaded area.
LPA conducted a review of (4) client records, (8) staff records, (4) clients Personal & Incidental records and reviewed the facility disaster plan. Staff records were complete with required documents and training. Client records were complete with required documentation. The facility disaster plan was current and in compliance with Title 22 at the time of visit. LPA reviewed (4) client medication administration records and medication and did not observe any discrepancies at the time of visit.
At 10:00am, LPA and staff toured the facility. There are no bodies of water or firearm/ammunition on the premises. All client rooms were checked. Beds and bedding were in good condition, adequate lighting was provided, and adequate storage for client’s personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations.
