25 | On 04/02/25, Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Administrator Zoie Clarke as the purpose of the visit was explained. The facility is licensed to serve 4 develop disabled adults ages 18-59, the facility is approved for 4 ambulatory of which 2 may be non-ambulatory. Clients are linked to the South Central regional center. Facility fees are current, surety bond and liability insurance observed to be active.
The facility is a one-story home located in a residential neighborhood. The home consists of 3 client bedrooms, 2 bathrooms, 1 living room, 1 office area, 1 kitchen, 1 detached garage that is used for storage, houses a washer and dryer and a deep freezer. There is a big backyard patio area with shaded seating, facility is currently planting flowers and plants in the backyard, there are no bodies of water were observed, no security bars or weapons on the premises. Client bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there are sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F.. A supply of perishable and non-perishable food was observed, toxins and knifes were stored and inaccessible to clients.
LPA conducted a records review of 2 staff records, 4 client records, 4 medication administration records, and 4 P&I ledgers, no discrepancies observed. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 03/01/25, 1 fire extinguisher fully charged, carbon monoxide and smoke detectors are interconnected and operational, administrator was advised to check batteries regularly.
