25 | On 7/10/2024, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Zachary Howell /Executive Director. LPA explained the purpose of today’s visit. The facility is licensed to serve (127) non ambulatory elderly adults ages 60 and above, of which (9) can be bedridden. The facility has an approved hospice waiver for (20).
The facility has 80 units, and approximately 86-bathrooms, is five stories tall with a basement garage. The facility is a beige in color structure with a gym/ physical therapy room, bistro, formal dining room, theater, a restaurant style kitchen, and two elevators. There is a large patio area on the 3rd floor and other sitting areas on the 1st floor.
LPA Iniguez and the Executive Director toured the physical plant. There were no bodies of water or obstructions on the premises. LPA inspected a total of (10) bedrooms and (10) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 113.5°F to 115.2°F, and the room temperature ranged from 75°F to 76°F.
