25 | On 4/9/26, at 1:12pm, the department conducted a case management visit to the facility and was greeted by Ashley Acosta, Administrator. The department explained the purpose of this visit was to conduct a final walk-through to close the facility and to retrieve the Department of Social Services Community Care Division License.
The department conducted a tour of the entire facility. During the inspection, the department observed the following: (4) bedrooms rooms, (2) bathrooms (1) staff bathroom, a living area, a dining area, a kitchen, an office, an outside seating area, and garage. The department did not observe any clients in the facility and all client rooms were empty and their belongings were no longer at the facility. Acosta Family Home II submitted a closure plan to Community Care Licensing Division on 01/20/26, indicating the intent to close the facility and the eviction of all clients. The last of the clients were relocated to another facility on 03/07/2026.
The Administrator, Ashley Acosta, has voluntarily surrendered the Department of Social Services Community Care License effective 04/09/26 and will no longer hold a license to operate from Community Care Licensing Division for this facility. The administrator could not find the hard copy of the license. The department instructed the administrator to destroy the license if found, as it is no longer valid.
According to the California Code of Regulations (Title 22, Division 6, Chapter 1), the department did not observed deficiencies; therefore, no citations are issued. The facility is closed effective 04/09/2026.
An exit interview was conducted with Ashley Acosta, Administrator, and a hard copy of this Facility Evaluation Report was provided.
