25 | On 09/25/25 Licensing Program Analyst (LPA) Villegas conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Licensee Yolanda Stowe and the purpose of the visit was explained. The facility is licensed to serve (5) ambulatory developmentally disabled or Mentally ill adults ages 18 through 59. The facility currently has (0) clients in care. Facility fees are current.
The facility is a one story home located in a residential neighborhood, and consists of the following: 3 client bedrooms, 1 common bathrooms, living room, kitchen, dining area, family room that houses a washer and dryer. There are (2) additional bedrooms, (1) bathroom, (1) fire extinguisher, and a common space located in the ADU. LPA observed a shaded gazebo area in the backyard, no bodies of water nor firearms are on the property.
LPA toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly. Shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to future clients. The water temperature properly measured between 105F-120 F..
Posted & readily available for review in den area is the emergency disaster plan, emergency phone numbers, daily schedule, activities calendar, menu, clients rights, administrator certificate and visitors policy. (1) fire extinguisher is mounted in kitchen/den area.
Exit interview conducted with Yolanda Stowe, Administrator and a copy of this report was provided.
