On 07/01/2025, at 10:20 a.m., the California Department of Social Services Community Care Licensing Division (CDSS/CCLD) Licensing Program Analyst (LPA) Pamela Bunker conducted an unannounced initial visit to gather information regarding the above allegations. LPA Bunker met with Ibrahima Zayat, the Administrator and Noam Sakoff, the Assistant Administrator and explained the purpose of the visit.

The investigation consisted of the following: On 07/01/2025 at 10:20 a.m., the Department requested, reviewed, and obtained copies of the Client Roster (dated 07/01/2025), Personnel Report (dated 02/01/2025), Weekly Food Menu (dated 6/01/2025-07/31/2025), Weekly Laundry Schedule (dated 06/01/2025- 07/31/2025). Interviews were conducted with Staff Members 1-3 (S1-S3) and Clients 1-7 (C1-C7). At 12:30 p.m., S1 and S2, along with LPA Bunker, toured the kitchen, dining room, and laundry room. An ample supply of perishable and non-perishable foods was observed in the kitchen refrigerators, freezers, cabinets, and pantry. During the visit, staff were observed washing residents' laundry.

Investigation revealed the following:

Allegation 1: Staff do not provide residents with adequate food service.

On 07/01/2025, at approximately 11:30 a.m., interviews were conducted with facility Staff members 1 through 3 (S#1-S#3). All three staff members (3 out of 3) stated that residents are provided with adequate food service. They reported that residents receive three well-balanced meals per day, breakfast, lunch, and dinner, plus snacks, seven days a week, from Sunday through Saturday. 3 out of 3 staff stated that alternative meal options are available if residents do not want what is on the menu. 3 out of 3 staff stated residents are served a second helping if they request it. According to S1-S3, there have been no complaints from residents regarding the food being served. S1-S3 denied the allegation and stated it is false.

During the visit, LPA Bunker observed residents eating nutritious, well-balanced lunches and dinners that matched the facility's posted food menu for the day.

On 07/01/2025, at approximately 2:30 p.m., interviews were conducted with residents 1 through 7 (R#1-R#7). All seven residents (7 out of 7) stated that they receive plenty of food to eat and are served three meals a day, breakfast, lunch, and dinner, plus snacks.

Seven out of seven residents confirmed that: Meals are served seven days a week, from Sunday through Saturday. They are allowed to have second servings. They are not served a single piece of chicken with a small portion of mixed vegetables. Hot dogs are not served daily for lunch and dinner. Breakfast options are not limited to only unsweetened oatmeal, dry cereal, or one pancake. Coffee is served during breakfast, lunch, and dinner. 7 out of 7 residents stated that they had no complaints about the food being served. 7 out of 7 residents denied the allegation that staff fail to provide residents with adequate food service.

Based on interviews, available evidence, observation, information received, and records reviewed there was not enough sufficient evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is deemed unsubstantiated. Allegation 2: Staff do not provide residents with adequate laundry service.

On 07/01/2025, at approximately 11:30 a.m., interviews were conducted with facility Staff members 1 through 3 (S#1-S#3). All three staff members (3 out of 3) stated that laundry is handled by more than two staff members, and it is done daily, from Sunday through Saturday, or as often as needed. Residents send their clothes in labeled bags to be washed, and they do not receive the wrong items. If a mix-up were to occur, staff would ensure it is corrected immediately. All three staff members (3 out of 3) stated the facility provides residents with adequate laundry service. According to S1-S3, there have been no complaints from the residents regarding their laundry or receiving incorrect items. All three staff members (3 out of 3) denied the allegation.

On 07/01/2025, at approximately 2:30 p.m., interviews were conducted with Residents 1 through 7 (R#1-R#7). All seven residents (7 out of 7) stated that laundry is handled by four staff members. Each resident stated they send their clothes in labeled bags to be washed, and their clothes are returned to them clean and folded. All seven residents confirmed that they do not receive the wrong items, and 7 out of 7 stated that they have had no complaints with their laundry. 7 out of 7 residents denied the allegation that staff do not provide adequate laundry service.

Based on interviews, available evidence, observation, information received, and records reviewed there was not enough sufficient evidence to support the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is deemed unsubstantiated.

There were no deficiencies cited. Exit interview conducted.
