32 | On 1/8/26 and 4/2/26 Licensing staff conducted unannounced visits and recorded observations. During visits LPA Mooberry observed the home was observed to be clean and orderly with no smell of smoke.
On 1/8/26 LPA Chambers observed 4 infants in care with 1 staff, the home was observed to be operating out of ration, the deficiency was addressed on a previous report.
Two witnesses interviews stated they are able to enter the home while their children are in care and did not corroborate the reported allegations. The reporting party was anonymous and not able to be contacted.
Based on interviews conducted during this investigation, observations made and records reviewed all four allegations are found to be unsubstantiated, Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur.
No deficiencies cited during inspection.
Exit interview was conducted with Elexis Longwood including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
