Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced annual/random inspection at the above facility. LPA did a RSO search before approaching facility. LPA was greeted by Staff Freddie Boatright at 1:14pm. At 1:15pm LPA Calderon called licensee and left a voice message. Ms. Boatright notified Licensee Carlos Taylor aware of LPA arrival. Licensee Mr.s Taylot allowed LPA entry into the home at 1:20pm.

Mr. Taylor guided analyst on a tour of the facility. There were 3 infant and 8 pre school children present during today's inspection. Facility capacity is in compliance for a Large Family Child Care Home. Hours of operation are Mon-Fri 6:00 AM - 6:00PM. This is a one story home that consists of 4 bedrooms (1 bedrooms used for napping infants), 2 restrooms, kitchen, living room, den (day care room), garage, backyard, and front yard. Per licensee, the children use the restroom in the hallway, the day care room (den), living room, one bedroom, and backyard. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights. LPA provided (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 08/22/24.

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged and is located in the dining area, receipt shows purchase date 1/31/23, deficiency cited. LPA consulted Licensee's to purchase fire extinguisher yearly or get extinguisher serviced yearly per State Fire Marshall Regulations. The home maintains telephone service via cell phone. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. (Pg 1/3)

LPA observed that detergents, cleaning compounds inaccessible to children in kitchen cabinets, baby safety locks in place. Licensee understands that all poisons must be locked, currently poisons are stored in locked garage. Isolation area for sick children waiting to be picked up is in the living room, away from the other children. Per Licensee there are no firearms or weapons stored in the home. Sharps are locked in the kitchen in high cabinets inaccessible. Per licensee(s), food is provided for the children. Per licensee, facility has a pet that does not interact with children and placed in off limit area. LPA observed screened and barricaded fire place in living room.

Licensee cares for three infant. Napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. LPA did not observe loose object, bumpers, objects hanging, or objects attached to the play yards. LPA observed at 1:25pm (2) infants with attachment on pacifiers, Child #3 and Child #4, deficiency noted. Mr. Taylor removed attachments from infants. Each infant has their own play yard and bedding. LPA gave license 15 min sleep log sample for infants. currently licensee tracks sleep log in a notebook. Licensee does not provide any overnight care at this time.

The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that could be hazardous to children in care. There are no pools or spas, or other bodies of water.



(5) Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC9227 Individual Sleep Plan (0-12 months), and 15 minute Infant Sleep Check (0-24 months). (4) Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. Certifications for Licensee's were active/ valid.

At approx 2:03pm Staff #4 arrived at the facility to assist with day care children. At 2:45pm Licensee Crystal Stringer arrived at the facility. LPA gave consultantation on staff records, informed licensee on LIC311D, and on Entrance Checklist FFC homes. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided.

  • Licensee was reminded: all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
  • LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
  • Incidental Medical Services (IMS) policy was discussed. For IMS info. see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.
  • Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on today’s visit, Per Title 22 regulation there were deficiencies provided on LIC 809-D, appeal rights given. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee Crystal Stringer.

LPA gave PIN 20-24-CCP & infant safe sleep regulation 102425 to review. Licensee will write declaration statement on reviewing infant safe sleep regulation and pin and state (3) things they learned as part of training and have staff sign as well. Licensee will send documentation's to LPA Calderon by poc due date.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
