25 | Licensing Program Analyst (LPA) Susann Sanchez conducted an unannounced case management inspection to follow up on an incident that was reported to the Department on 01/25/2023. Upon arrival, LPA met with Licensee Crystal Stringer, who provided LPA a tour of the facility inside and outside. Census was taken.
On 01/25/2023, an incident was reported to the department where a child was pushing a Little Tikes car/scooter and then turned away from the car/scooter causing the child to miss their step. The child then fell, landing on their elbow. On 01/24/23, Parent #1 informed Licensees that child #1 had a broken arm. This incident occurred on 01/23/23. The facility reported this incident to the Department within the required 24 hours of them knowing the child broke their arm. During this inspection LPA interviewed Licensee Taylor who observed incident and Licensee Stringer. LPA observed the outdoor space where the incident took place and took photos. LPA obtained and reviewed documentation. Based on the information obtained and LPA observation of the play area, LPA determined there was adequate supervision provided the day of the incident. LPA observed the Unusual Incident written report and Licensee Stringer stated she is still gathering information and will submit written report to LPA by Friday 01/31/2023.
There is no deficiency cited during today's visit. Exit interview conducted with the Crystal Stringer.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
