Licensing program Analysts Alicia Mooberry and Andrea Carter conducted an unannounced inspection to the above facility.

Upon arrival at 3:00pm, LPAs met with facility representative Olivia Gibson, Teacher for Preschool license on site. LPAs observed 3 children without the dedicated supervision of staff. Licensee arrived at 3:25pm bringing the facility into compliance.


Olivia Gibson stated they were visually supervising from the room next door which is the pre-school license. There is a half door separating the school age license from the preschool.

Licensee was advised that the lack of supervision of children is an immediate risk to the health and safety of children in care.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee/Director was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee/Director.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee, Teliscia Martin. Appeal Right were discussed and provided

The director of the child care center component within the combination program shall maintain administrative responsibility for the overall program and shall provide direct supervision and guidance to the school-age child care center component.

The licensee arrived to the faciltiy at 3:25pm bringing the facility into compliance. Per licensee, they will make sure to stay at the facility to provide supervision

Based on LPA observations, there were 3 children in School-age classroom without a dedicated staff supervision posing an immediate health, safety and personal rights risk to children.
