On 3/18/25 at 11:20 am Licensing Program Analyst Claudia Kam conducted a Case Management incident inspection at the above facility to follow up on Plan of Corrections.

LPA completed record review. The purpose of today's inspection was to review files and documentation to clear a plan of correction.

LPA observed fire drill, fire permit, toddler furnishings for deficiencies on 11/1/2024. Deficiency has been cleared and letter provided to facility representative.

Based on this information the following deficiencies on the LIC 809 D are being cited today 3/18/2025 for report 1 of 2.

LPA Claudia Kam informed director Peyton Solomon that this report dated 3/18/2025 documents one (1) Type A citation which shall be posted for 30 consecutive days as there are immediate risk to the health, safety, or personal rights of children in care.



Also, LPA Claudia Kam informed the facility representative to provide a copy of this licensing report dated 3/18/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Exit interview was conducted and report was reviewed with the facility representative Yadira Godlock and Peyton Solomon.
