On December 27, 2024, at 1:04 PM, Licensing Program Analysts (LPAs) Ashley Calderon and Tyler Reyes, conducted an unannounced Annual/ Random Inspection for the preschool license. During today’s inspection, LPA met with Interm Director Shaniece Smith and also present facility representative Crystal Cyprian who granted LPA's entrance to the facility.

LPAs conducted a tour of the facility alongside with S.Smith. Facilities hours of operation are Monday through Friday from 7:00 AM – 6:00 PM. This facility consists of 2 classrooms and outdoor play area. All areas identified on the Facility Sketch were inspected. Facility is located on the premises of Family of Prayer and Praise Church.

LPAs observed two classrooms, Room 1 and Room 2. LPA's observed 3 children in care and 3 staff in classroom #1, facility is within ratio and license capacity. Per Interm Director, currently room #2 is not being used due to number of children enrolled. Staff present are all fingerprinted cleared. There was 1 staff present at the facility with a valid pediatric first aid and cpr.

The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are being provided by facility and on Friday's children parents take linen home to get washed and facility provides washing services when needed. LPAs observed napping cots are located in classroom. Age appropriate sinks and toilets were inspected for availability and good repair in all classrooms.Drinking water via water dispenser was observed in classrooms. Per S.Smith food service is conducted via food vendor Children's Nutrition Express Company for: breakfast, lunch, and afternoon snack. Per department report dated: 11/3/23 facility representative stated: No water used for food preparation or washing. Per Interm Director Shaniece Smith stated on todays visit uses kitchen sink to washing dishes and no food touches the kitchen sink water. LPA Calderon will follow up with facility regrading requirements for lead.

Carbon monoxide, smoke alarms and fire pull system detectors are interconnected and hard wired with Los Angeles Fire Department. Fire extinguisher has a inspection service tag dated July 13, 2024. Last disaster and fire drill was conducted on 12/2/24.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. LPAs observed children water bottles available for water accessibility. There are no pools or bodies of water at the facility.

Disinfectants, cleaning solutions, and other items that are dangerous to children, were inaccessible to children placed in high inaccessible cabinets or under sink located in kitchen area with safety lock. No medications observed at the facility, per S.Smith currently no children on medications. if medication is administrated at the facility medications will be stored in the Director's office in a inaccessible area.



Menus are posted in each classrooms. All kitchen, food preparation, and storage areas were observed clean and free of litter/ rubbish. All food is protected from contamination. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

Documents to be posted were posted in each classroom including Facility License, wavier was posted for Playground, there is staggered schedule for outdoor play.

On today's date it was reported that previous Director LaTonya Patterson is no longer with the facility as of 12/18/2024. The name of the child care center director designated to act in the director’s absence has been reported to the Department, the person is Interm-Director Shaniece Smith. LPA reviewed S.Smith staff file for qualifications. LPA reviewed Site Supervisor/Teacher David Samano file an additional director designated to act in the director’s absence. Facility is in the process of hiring a full term Director. Designated staff have proper qualifications in file.

LPAs reviewed staff files for staff present and child records for children present, all records were observed in file.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Per California Code of Regulation Title 22, no citations were cited on today's visit.

During the exit interview, the facility representative Shaniece Smith, confirmed that there are no Registered Sex Offenders in the facility and LPA completed the RSO profile in FAS. A notice of site visit was provided to facility representation must remain posted for 30 days. Exit interview conducted and report was reviewed with facility representative Shaniece Smith.
