Licensing Program Analyst (LPA) R. Derraco conducted an unannounced complaint inspection to the above mentioned facility on 09/30/25 at 9:30 AM. LPA was met by licensee, Erindera Doherty, who guided analyst on a tour of the home. LPA observed 9 children in care, 2 of which were infants. There were two assistants helping with the children in care. Adults who reside in the home were noted and discussed. All adults in the home were observed to have criminal background clearance and associated to the facility. The home was observed to be clean and free of defects.

The purpose of this visit is to deliver findings to the above mentioned allegations. During the course of the investigation, LPA conducted interviews, reviewed records, and made observations. Individuals interviewed understand and practice safe sleep protocols when handling infants. Infant sleep charts were observed to be updated and complete and cribs infants use to sleep in were observed to be in compliance with title 22 regulations. Bed sheets in each crib were observed to be clean. Individuals interviewed state that spare sheets are available for the cribs if and when they become soiled. LPA observed additional sheets stored in

the closet. Individuals interviewed state that the door always remains open when infants are napping in their respective rooms. LPA observed napping mats to be free of defects and in good repair. Linens are provided by the day care and used to cover the napping mats when used by children during nap times. Individuals interviewed state that linens and crib bedsheets are laundered once a month or as needed if the linens are soiled. LPA observed the outdoor play area to have several age appropriate toys and outdoor play equipment. Extension cords were observed to be tied to a tree and inaccessible to children in care. LPA observed the assistant cleaning the floors and the tables of the day care facility throughout each visit. Individuals interviewed state that there are no concerns with the physical plant of the facility, noting that the home is clean. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

A notice of site visit was given and must remain posted for 30 days.
