32 | Children’s records were reviewed for Emergency Card with signed medical consent, Immunization Records, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement.
Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against Measles, Pertussis and Influenza or Influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. LPA observed 2 out of 2 staff missing Pertussis and Influenza or influenza declination immunizations and 1 out of 2 staff did not have Measles immunizations at the time of inspection, this poses a potential risk to the health and safety of the children in care. 1 out of 2 staff did not have the Mandated Reporter Training Certificate at the time of inspection. 1 of 2 staff did not have a current Pediatric First Aid and CPR certification at the time of inspection.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
