On 10/20/22 at 9:20 AM Licensing Program Analysts (LPAs) Austin Estrada and Jeanette Estrada conducted an unannounced Annual Required inspection. LPAs met with Director Linda Johnson who assisted LPAs with a tour of the facility. All staff present for the preschool program have obtained a criminal record clearance. This is a Preschool program. On the same premises there is an Infant/ Toddler component (facility #198400297). The Preschool program operates on the grounds of a Church building. The preschool program is separate from the church and consists of three classrooms. Per Director, only one classroom is being used for the time being due to the number of children enrolled. Hours of operation are Monday through Friday 6:30 AM to 6:00 PM. Upon arrival there were 2 children with 1 staff present. At 10:05 AM LPAs stepped out of the facility due to the California great shake-out 2022. LPAs continued report at an alternate location. LPAs returned to the facility at 11:10AM. Upon arrival, there were 3 children total in the preschool program.

All areas identified on the Facility Sketch were inspected. Currently there are four children enrolled in the Preschool program.

LPAs observed a greeting station with sign-in sheets located at the entrance of the building. Two of three children present were signed in. LPAs advised Director that all children present need to be signed in and out daily. LPAs toured the preschool room and observed exposed electrical wires coming from the wall near the entrance of the classroom. The electrical outlet with exposed wires was observed to not be covered and poses an immediate health and safety risk for children in care. LPAs observed a trashcan in the preschool room that did not have a tightfitting cover at the time of inspection. LPAs advised the director that all trashcans shall have a tightfitting cover to ensure the health and safety of children in care.

Furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. Napping equipment and bedding were inspected. The linens are washed weekly by staff or as needed. Inspection was conducted for appropriate storage and cleanliness of children's blankets and personal items. Each child had their own cubbies and storage area. LPAs observed water containers in the classroom with cups labeled with children’s names. Water was readily available to children in care. Per Director there are not any current children that take medication. If a child needs medication it will be stored in the Directors office inaccessible to children in care. There is a restroom located outside the preschool classroom. The restroom had three toilets and two sinks all in safe and sanitary operating conditions.

Per Director there are no poisons located at the facility. LPAs advised the Director that if poisons are to be stored at the facility they must be locked and inaccessible to children in care. Per Director the “Chruch Overflow Room” is the isolation area for ill children waiting to be picked up by parents/ guardians. Parents are contacted immediately when children are determined to be ill. Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides breakfast, lunch, and afternoon snacks. Kitchen areas, food preparation areas, and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. Per Director, children usually eat their meals in their classroom, but may eat outside on occasions if weather permits. LPAs inspected the food preparation area and refrigerator and observed it to be clean and in operable condition. This space is also shared with the church. LPAs observed items in the refrigerator. Food was observed not to be expired. Food that was not in the original packaging were sealed and labeled with an expiration date.

Disinfectants and cleaning solutions are inaccessible to children and are stored in the Directors office. The facility did not have a functioning carbon monoxide detector on the premises. LPAs observed a central fire alarm system installed in the facility.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Areas around and/or under climbing equipment were observed to have cushioning material to absorb a fall. LPAs observed a storage door near the outdoor play area to be unsafe for children in care. Door did not have a doorknob and had had nails protruding from the door which is a potential health and safety risk for children in care. Director states that she will contact the church to get the door fixed and will put a gate up to block children from the surrounding area. LPAs did not observe any bodies of water during this inspection.

Children’s records were reviewed for Emergency Card with signed medical consent, Immunization Records, LIC 995 Notification of Parents’ Rights, LIC 701- Physician’s Report, LIC 613A- Personal Rights, and signed Admissions Agreement.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 9052- Employee Rights, Proof of immunization against Measles, Pertussis and Influenza or Influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse, Transcripts or Permit and current Mandated Reporter Training Certificate. LPA observed 2 out of 2 staff missing Pertussis and Influenza or influenza declination immunizations and 1 out of 2 staff did not have Measles immunizations at the time of inspection, this poses a potential risk to the health and safety of the children in care. 1 out of 2 staff did not have the Mandated Reporter Training Certificate at the time of inspection. 1 of 2 staff did not have a current Pediatric First Aid and CPR certification at the time of inspection.



Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited include one Type A violation and three Type B violations. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. The Director was provided with LIC 9224 Parent Acknowledgement of Report Received.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.



The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview conducted and report was reviewed with Director Linda Johnson. Director was provided a copy of their appeal rights (LIC 9058).
