Licensing Program Analyst (LPA) Denise Gibbs conducted an announced pre-licensing inspection on 7/27/21 at 10:00 AM. LPA met with Dr. Linda Johnson, Applicant who guided analyst on a tour of the facility.

This is a proposed Preschool Program which consists of Three classroom. This program will serve children ages, Two to six Years Old. Operating hours will be Monday - Friday 6:30AM - 6:00PM. Facility plans to also house an Infant component with a Toddler option, facility #198400297.

All areas identified on the facility sketch were toured both indoors and outdoors. Measurements of the indoor and outdoor space were taken as needed.

Required postings will be visible in the entrance of each classroom. Due to COVID-19 precautions documents will also be posted in parent drop off area in the main entrance. LPA observed all required documents printed. Sign-In and Sign-Out will be conducted by use of the Bright Wheel application. If application is down, manual sign in/out will be used. Health checks will be conducted before children enter the facility. Temperatures will be taken daily for COVID-19 precautions.

LPA observed three classrooms, One Preschool Room and two Kindergarten Rooms. Furniture and equipment was inspected for age appropriateness and good repair. Each classroom was observed to have age appropriate materials. LPA observed a music area in the preschool room. LPA advised applicant child proof the plastic edges as this could be a possible safety hazard. Telephone service, heating, lighting and ventilation were evaluated and are operable. Children will have their own cubby to store their belongings. Linens will be washed weekly by the facility representative or more frequently as needed. Linens will be provided by the facility with multiple sets on hand.

LPA observed two restroom, one near the Preschool Room and one near the Kindergarten Rooms. Restrooms were observed to be clean with four working toilets and three working sinks.

LPA observed that facility has an operable carbon monoxide detector, smoke detectors, and serviced fire extinguishers. Drinking water will be provided using water jugs and cups both indoor and outdoors.

Disinfectants, cleaning solutions, and other items that are dangerous to children will be in an inaccessible in a locked storage room. All storage containers for solid waste have tight-fitting covers that are kept on and are in good repair.



Per applicant, the isolation area is located in the front section of the overflow room near reception area with walking access to the staff restroom; there will be a cot available for children to rest if needed. As of now applicant states that short term medication will not be administered at the facility due to COVID-19 precautions, this may change in the future. First aid kits are available in each classroom and in the main entrance to the facility.

This program will provide Breakfast, Lunch and PM snack. Meals and snacks will be delivered daily from a partnered restaurant in individual servings and containers. Extra snacks will be available on hand at the facility. A monthly Lunch/Snack menu is available and will be posted. LPA informed Applicant that all bags and/or dishes that are brought from home must be labeled with the child’s name and date for proper storage. Per applicant kitchen and food prep areas will rarely be used due to catering service. All kitchen areas/food preparation areas and food storage areas were observed to be clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. The facility was observed to be free of flies, other insects and rodents.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. LPA advised applicant that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. LPA observed tented area outside of the facility which provides a shaded area. There are no bodies of water or firearms on the premises. LPA observed a shaded area, three grass areas and a cement area for children to play. LPA advised that applicant add two safety gates outdoor, one leading to the parking lot and one leading to a blind spot behind the play area. Per applicant they will add barriers for safety.

Incidental Medical Services (IMS): This facility plans to provide Incidental Medical Services – IMS. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. A Plan of Operation that includes IMS must be submitted to the Department.

An IMS Plan was provided with the application and will be reviewed.

Additional Items discussed during inspection: COVID-19 self-assessment and precautionary procedures. Applicant provided LPA with completed self-assessment on 7/27/2021. The following was discussed:

Essential Protective Equipment and Supplies- Applicant has a contact-less thermometer and a supply of disposable face coverings, gloves, hand sanitizer and disinfectant cleaning supplies at the facility. Children and adults will be required to wear facial coverings throughout the day.


Cleaning- Per applicant, there is a dedicated staff that will work throughout the day to disinfect high touch areas and cleaning throughout the facility. Teacher's assistants will also help with this task.
Hygiene- Per applicant hand washing will be a part of the curriculum that they will be working on throughout the day. Hand sanitizer will be available when they cannot get to the sink right away. Per applicant personal items will by limited, hygiene items will be provided by facility. Lunched will be provided in individual servings. The isolation area for a sick child waiting to be picked up will be in the overflow room near the exit.
Arrival Procedures- Per applicant parents will drop off/pick up in the covered patio area. New parent will be able to talk to teachers by appointment. Staff will do daily temperature checks and health screening at the front door, as parents will not be allowed to enter the facility. COVID-19 posters and resources are posted in the front entrance accessible to parents. Parents will be made aware of the COVID-19 policies and exclusion procedures initially through the enrollment process, the parent handbook and a group orientation before children begin.
Distancing/Meals/Sleep/Outdoors- Per applicant they will be doing a lot of outdoor activities. They will practice distancing and implement activities into the curriculum so that distancing will be fun. They will have staggered play throughout the day where small groups will be inside and small groups will be outside. For meals they will also use the outdoor as much as possible. When indoors meal times are needed staff will reinforce social distancing. Children can be spread out for nap at least three feet.

Applicant is seeking to provide care for 30 Preschool; Based on measurements and fire clearance, the facility has room to care for 30 Preschool children. A fire clearance has been granted as of 7/9/2021.

Based on the LPA observation, the following corrections need to be made prior to obtaining Preschool license.

  • Addition of two safety barriers in the Preschool yard
  • Child proof edges for music area in Preschool Rooms


A Child Care license may be granted upon receipt of proof of corrections and Licensing Program Manager (LPM) Approval. Once licensed, the applicant is required to comply with the terms and limitations stated on the license.

Exit interview was conducted with Dr. Linda Johnson, applicant. Applicant was given a signed copy of this report.
