Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert conducted an unannounced complaint inspection. LPA met with Licensee Dr. Linda Johnson and informed her of the reason for the visit. Upon arrival LPAs observed four infants with one staff in the infant room and five toddlers and two staff in the toddler room.
At approximately 10:25 AM, LPAs entered the infant classroom and observed five infants with one staff. Interviews conducted confirmed that the infant classroom is out of ratio at times. Staff interviews also revealed that Staff 1 assists in the classroom if needed. Staff 1 was confirmed to not meet the educational requirements to assist with providing care. During today's inspection, one infant was picked up to meet ratio requirements
The preponderance of evidence has been met, therefore the above allegation is found to be substantiated.
A type A deficiency is being issued today for 101416.5 Staff-Infant Ratio. California Code of Regulations, (Title 22, Division 12, Chapter 1), are being cited on the attached LIC 9099D

Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert conducted an unannounced complaint inspection. LPA met with Licensee Dr. Linda Johnson and informed her of the reason for the visit. Upon arrival LPAs observed four infants with one staff in the infant room and five toddlers and two staff in the toddler room.
LPAs observed Licensee Dr. Linda Johnson in the toddler classroom with Staff 1. LPAs were provided with a transcript for Staff 1. Transcript was not clear. Licensee verbally confirmed that Staff 1 does not meet the educational requirements and was observed assisting with care on 3/26/24 and today, 4/9/24. During today's inspection, Licensee called parents to pick up children in the toddler classroom.
The preponderance of evidence has been met, therefore the above allegation is found to be substantiated.
California Code of Regulations, (Title 22, Division 12, Chapter 1), are being cited on the attached LIC 9099D.
Exit interview conducted and a copy of the report and appeal rights were provided to the Licensee, Dr. Linda Johnson. The notice of site visit was posted where the parent/guardian of children enter and exit the facility.

Licensee decided to close the toddler classroom during today's visit. Licensee has contacted two staffing agencies to provide additional qualified staff. Licensee will provide proof of contract with staffing agencies by 4/10/24.

Based on record review and interview, Licensee did not ensure that Staff 1 met the educational requirements prior to hiring them as an assistant in the facility. LPAs observed Staff 1 providing care in the toddler classroom. This poses a potential threat to the personal rights of children in care.

A copy of this report shall also be posted where the parent/guardian of children enter and exit the facility. Both the notice of site visit and licensing report shall remain posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.
