On 1/10/23 Licensing Program Analyst (LPA) Jeanette Estrada conducted a case management visit for the purpose of amending the LIC 809 report from 10/20/22 and completing the annual continuation inspection.
LPA met with Director Linda Johnson who guided analyst on a tour of the facility. Present during today's inspection were 3 children with 1 staff in the infant classroom and 2 children and 1 staff in the toddler classroom.
During today's inspection LPA completed the required 1 year annual inspection and issued deficiencies observed on the visit conducted on 10/20/22.
LPA issued six type B citations.
-The facility did not have a carbon monoxide detector. On today's visit, LPA observed an operable carbon monoxide detector in the hallway.
-LPA observed the crib and play yard in the infant classroom contained blankets and toys. LPA also observed a play yard positioned underneath an object mounted on the wall. On today's visit LPA observed the items removed from the crib and observed the play yard had been moved out from underneath the wall mounted object.
-Records for Staff 1 and Staff 2 did not contain a Mandated Reporter certificate. Today LPA verified the mandated reporter for child care providers training has been completed. Staff 1 completed it on 10/21/22 and Staff 2 on 10/23/22.
-Records for Staff 1 and Staff 2 did not contain proof of immunization against measles, pertussis or flu. Today LPA verified that both staff records contain the proof of immunizations.
-Staff 3 was not associated to the license. Staff 3 has been associated to the facility.
-Childrens records: 3 out of 3 infant records reviewed on 10/20/22 did not contain the 15 minute infant sleep log.
