25 | Licensing Program Analysts (LPAs) Jeanette Estrada and Jonnisha Culbert conducted an unannounced case management visit to the facility. LPAs met with Licensee Dr. Linda Johnson. LPAs observed a total of five infants with one staff in the infant room and five toddlers with one qualified staff in the toddler room. Over ratio in infant room was cited on LIC 9099 dated 4/9/2024.
A citation is being issued for a child enrolled in the toddler classroom observed in the infant classroom during a visit on 3/26/24. LPAs reviewed records which revealed that Child 1 is over the age of two. The Infant classroom is licensed for children ages zero to two years old.
During today’s visit, LPAs observed two cribs in the infant room. Cribs were observed to have blankets and other items inside of them. Per Staff 5, Child 2 who was present uses one of the cribs to nap. Per Staff 5, the infant who uses the other crib was not present today. LPAs did not observe infants in the cribs at this time. Per Staff 5, the rest of the infants present do not use the cribs to nap. LPAs observed Child 3 in the classroom and inquired with Staff 5 where child 3 sleeps. Per Staff 5, Child 3 naps on a big pillow in the classroom. There is a separate napping room in the facility which is designated as the infant napping space. Per Licensee and Staff, the napping room is not being used by infants and is being used by the toddlers for napping. Infants are currently napping in the classroom.
At approximately 10:25 AM, LPA J. Estrada entered the classroom and observed Staff 5 tilt forward an infant rocker which had Child 4 sitting in it. LPA observed Child 4 fall forward onto the carpet.This is an immediate risk to the personal rights of children in care. LPA inquired with Staff 5 about tilting the rocker forward while Child 4 was in it. Per Staff 5, the children use the rocker not as it is intended and swing side to side from it and she lifts the rocker forward to get the children off. LPA advised to have children use the rocker as intended and if they need to be taken off, to gently help the children. LPA also advised to remove the rocker from the classroom if it cannot be used appropriately. Staff 5 removed the rocker from the classroom.
